Zusammenfassung
Die Prävalenz der Osteoporose nimmt bei Menschen ab dem 50. Lebensjahr erheblich zu. Bei sonst gesunden 60-Jährigen haben etwa 5% der Männer und 10% der Frauen eine manifeste Osteoporose, bei 80-Jährigen liegen die entsprechenden Werte bei 15% bzw. 35%. Chronische Erkrankungen, die sich auf den gesamten Organismus auswirken, werden oft von einer Osteoporose begleitet. Unter diesen kommt der COPD eine besondere Bedeutung zu, leiden doch 35–70% der Patienten mit COPD unter einer Demineralisierung der Knochen. Das betrifft auch schon COPD-Patienten unter 50 Jahren. Ursächlich dafür sind die vielfältigen Auswirkungen dieser Erkrankung auf den Gesamtorganismus. Zudem kann die Therapie mit Glukokortikoiden Einfluss auf den Knochenstoffwechsel haben. Ausschließlich eine Messung der Knochendichte gestattet eine sichere Diagnose. Die Basis der medikamentösen Therapie einer Osteoporose sind Kalzium und Vitamin D. Erweitert werden kann die Behandlung bei Bedarf durch Bisphosphonate.
Abstract
The prevalence of osteoporosis increases in humans above the age of 50 years. In otherwise healthy people, 5% of men and 10% of women have significant osteoporosis, and these percentages increase to 15% and 35%, respectively, at the age of 80. Chronic diseases affecting many organ systems of the body are frequently accompanied by osteoporosis. This is of particular interest in patients with chronic obstructive pulmonary disease (COPD), since 35–70% of such patients have some degree of bone demineralisation. This might also affect COPD patients younger than 50 years, due to the numerous impacts of COPD upon other organ systems. Treatment with systemic steroids might further aggravate this problem. Measurement of bone mineral density is the only way to assess the diagnosis. Supplementation with calcium and vitamin D is the basis of treatment. In addition, therapy with bisphosphonates may be necessary.
