Résumé
Les fractures du pilon tibial sont peu fréquentes, elles présentent un haut risque de complications précoces et tardives comme les cals vicieux et les pseudarthroses. Ces deux complications sont très mal tolérées, leur prise en charge reste un problème d’actualité en raison de leur mode de correction, de fixation et de difficulté de consolidation. Elles surviennent souvent chez des patients multiopérés avec un mauvais état cutané. Nous rapportons deux cas de cals vicieux supramalléolaires que nous avons traités par le fixateur externe d’Ilizarov dernière génération : le Truelok® et Truelok Hexapodal® assisté par un logiciel de planification préopératoire et de suivi. Le recul actuel est de deux ans.
Abstract
Pilon fractures of the distal tibia are not very common, but they are at high risk of early and delayed complications such as bone calluses and pseudarthroses. These two complications are very poorly tolerated and their treatment remains an ever present problem due to their method of correction, fixation, and difficulties with healing. They occur most often in patients who have had multiple procedures and whose skin is in poor condition. We are reporting on two cases of supramalleolar bone calluses that we have treated using the latest generation of Ilizarov external fixation apparatus: the Truelok® and the Hexapodal Truelok® assisted by pre-operative planning and monitoring software. The current timeframe is two years.
