Calcaneal metastasis of renal carcinoma: about a case and review of the literature

Résumé

Les acrométastases aux os du pied sont extrêmement rares. Nous présentons le cas d’un homme âgé de 73 ans suivi pour un carcinome rénal à cellules claires d’emblée métastatique au niveau pulmonaire. Il a été opéré par néphrectomie élargie. Cinq mois plus tard, il a consulté en urgence pour une douleur du pied gauche avec impotence fonctionnelle. Les explorations étaient en faveur d’une métastase calcanéenne d’un carcinome rénal. Notre patient a subi un curetage tumoral suivi d’une cimentoplastie, puis il a eu une radiothérapie de consolidation, et on lui a proposé une thérapie ciblée. Actuellement, il est encore vivant avec un recul de 14 mois. À travers ce cas rare de métastase calcanéenne d’un carcinome rénal, nous proposons d’étudier les particularités cliniques, radiologiques et thérapeutiques de cette entité rare ainsi qu’une revue de la littérature.

Abstract

Acrometastases of the bones of the foot are extremely rare.We are presenting the case of a male patient, aged 73 years who is being monitored for clear renal cell carcinoma with metastases to the lungs. He underwent a total nephrectomy procedure. Five months later, he was seen as an emergency due to pain in his left foot and functional impairment. Investigations highlighted calcaneal metastasis of the renal carcinoma. Our patient underwent tumour resection followed by cementoplasty then radiotherapy treatment; he has now been offered targeted therapy. He is currently still alive with a follow-up of 14 months. As a result of this rare calcaneal metastasis of a renal carcinoma, we are proposing to investigate clinical, radiological and therapeutic aspects of this rare entity as well as a review of the literature.
