Zusammenfassung
Hintergrund
Tumorpatienten sind häufig von einer Mangelernährung betroffen. Die Folgen sind eine Verschlechterung von Lebensqualität und Leistungsfähigkeit, geringere Toleranz gegenüber einer Tumortherapie sowie eine geringere Überlebenszeit.
Ergebnisse
Die frühzeitige Erkennung und Therapie einer Mangelernährung ist dementsprechend ein zentraler, relevanter Bestandteil der Tumortherapie. Zur Diagnose der Mangelernährung existieren strukturierte Fragebögen. Auch kommen objektive Methoden wie die Bioimpedanzanalyse zum Einsatz. Zur Behandlung der Mangelernährung hat sich in den letzten Jahren ein Stufenschema etabliert, das ausgehend von der Ernährungsberatung über die Verordnung von Trink- und Zusatznahrung bis zur Durchführung einer künstlichen Ernährung reicht.
Schlussfolgerungen
In klinischen Studien konnte gezeigt werden, dass eine Ernährungstherapie bei Tumorpatienten den Gewichtsverlust verringert und den Ernährungszustand stabilisiert. Leistungsfähigkeit und Lebensqualität sowie die Überlebenszeit nehmen zu.
Abstract
Background
Cancer patients frequently suffer from malnutrition resulting in an impaired quality of life, they are less therapy tolerant and have a poorer overall prognosis.
Results
Early diagnosis and therapy of malnutrition is, therefore, a central and relevant component of cancer therapy. Several questionnaires exist to facilitate the diagnosis of malnutrition. In addition, objective methods, such as bioimpedance analysis are used for assessment of the nutritional status. For therapy a cascade approach is applied, starting with nutritional counseling followed by sip feed and nutritional fluid supplements, with enteral and parenteral nutrition as the final options at the top of the cascade.
Conclusion
The results of clinical studies could show that nutritional therapy is able to reduce weight loss and to stabilize nutritional status. Physical fitness and quality of life improve and survival time increases.
