Zusammenfassung

Hintergrund

Dank immer weniger invasiver Operationsmethoden sowie einer gezielten postoperativen Lokaltherapie konnte die Anzahl von Komplikationen nach Katarakt-Operationen deutlich gesenkt werden. Die jahrelang eingesetzte postoperative Kombinationstherapie aus Diclofencac, Tobramycin und Dexamethason wurde an der Augenabteilung Hietzing im Juli 2012 zugunsten des neuen Monopräparates Bromfenac umgestellt.

Fälle/PatientInnen

Bei einer Gesamtanzahl von 1993 Eingriffen wurde bei acht PatientInnen ein klinisch signifikantes Irvine-Gass-Syndrom sowie bei vier PatientInnen ein Toxic-Anterior-Segment ähnliches Syndrom mit massiver Vorderkammerreizung diagnostiziert.

Schlussfolgerung

Die signifikante Steigerung dieser Komplikationen könnte auf die reduzierte Lokaltherapie zurückzuführen sein, allerdings lässt sich dies ohne weitere Studien nicht genau feststellen. Postoperative entzündlich bedingte Komplikationen nach Phakoemulsifikationen können jedenfalls nur durch in Frequenz und Behandlungsdauer ausreichende postoperative antiphlogistische Therapie verhindert werden.

Summary

Background

Owing to modern surgery techniques and adequate postoperative local therapy, complications after cataract surgeries have been minimised. The long-term application of local therapy after phacoemulsification was changed in July 2012 at the Department of Ophthalmology, Hietzing Hospital. Bromfenac, a new and easy to handle medication replaced the former combination therapy scheme consisting of Diclofenac, Tobramycin and Dexamethasone.

Cases/Patients

Out of a total of 1993 phacoemulsifications eight patients developed a clinically significant Irvine-Gass-Syndrome, four patients showed massive anterior chamber irritation similar to Toxic-Anterior-Segment-Syndrome postoperatively.

Conclusions

The reduction of postoperative medication could be the main reason of this significant increase in postoperative complications. However, this cannot be certainly assessed without further studies. In order to prevent postoperative complications a sufficient local therapy is essential.
