Zusammenfassung
Opioide werden neben ihrer bestimmungsgemäßen Verwendung zur Schmerzlinderung auch missbräuchlich eingesetzt. Hinweise dazu findet man u. a. im Internet. Unsere Untersuchung erfolgte zur Klärung der Fragestellung, ob und wie legal verordnete Opioide in der Missbrauchsszene Verwendung finden und ob es dabei spezifische Unterschiede zwischen den Präparaten gibt. Für eine Analyse des Missbrauchs verschreibungspflichtiger Opioide in einem deutschsprachigen Internetdrogenforum wurden 2010 und 2013 definierte Suchbegriffe in die Website-eigene Suchmaschine eingegeben und deren Ergebnisse ausgezählt. Dabei wurden die Häufigkeit der Substanznennungen sowie in der ersten Analyse Aspekte ihrer Inkorporation und Berichte der Nutzer zu unterschiedlichen Aspekten von Beschaffung, Anwendung, Wirkung und Gefahren untersucht. Das Opioid Tramadol war vor Codein das am häufigsten genannte Opioid. Danach folgten Tilidin, Morphin und Oxycodon. Andere Opioide erschienen in weniger als 10 % der Nennungen. Die orale Inkorporation wurde in Zusammenhang mit den genannten Opioiden am häufigsten erwähnt. Danach folgten die Injektion und die nasale Zufuhr. Die Studie soll klinisch tätige Ärzte, Pflegekräfte, Rettungsdienstmitarbeiter sowie Apotheker bei der Identifikation einer potenziell problematischen Einnahme von Opioiden unterstützen und das Augenmerk auf die am häufigsten verwendeten Substanzen lenken.
Abstract
Apart from the prescribed administration and indications for pain relief, opioids are also used for unintended purposes. Information for misuse is circulated on the internet. In order to analyze the abuse of opioids and opiates,which are only available by prescription, defined search keywords were entered into the search engine of a German language internet forum on drugs in 2010 and 2013 and the results were evaluated. Items to be assessed and analyzed were the frequency of naming various substances and (in the first analysis only) aspects of their incorporation as well as user reports on various aspects of use (e.g. drug procurement, administration, effects and side effects). Tramadol was the most frequently quoted opioid followed by codeine, tilidine, morphine and oxycodone. Other opioids were named in only 10 % of the entries. Oral intake was the most frequently mentioned mode of administration followed by parenteral and nasal routes. These findings can support caregivers to identify unintended use of opioids and to increase awareness of the most frequently used opioids and modes of administration.
