Zusammenfassung

Der Text geht vom psychiatrischen Alltag in Institutionen aus. Er zeigt zunächst, auf welchen Ebenen Psychoanalyse in der psychiatrischen Institution wirksam werden kann. Sie tut das, indem sie diagnostische und Behandlungsverfahren anbietet. Sie stellt aber auch eine Metatheorie zur Verfügung, die es erlaubt, den Alltag des psychiatrischen Handelns zu verstehen und therapeutisch nutzbar zu machen. Schließlich versteht sich Psychoanalyse nicht nur als Therapie in der Institution, sondern als Analyse der Institution, mit dem Ziel, die Institution selbst zu einem therapeutischen Faktor werden zu lassen. Der zweite Teil beschreibt die Voraussetzungen, die gegeben sein müssen, damit die Psychoanalyse auf den verschiedenen Ebenen fruchtbar werden kann. Zu unterscheiden sind dabei organisatorische Voraussetzungen wie Personalausstattung und Zeitmanagement von den konzeptuellen Voraussetzungen, etwa der Bereitschaft aller Ebenen der Hierarchie zur Mitarbeit oder zur Toleranz. Zu den Voraussetzungen gehören aber auch didaktische Maßnahmen und eine ausreichende personelle Ausstattung mit Psychoanalytikern. Die Arbeit schließt mit einem Appell an Psychoanalytiker, sich stärker der Psychiatrie zuzuwenden.

Abstract

This article reflects on the role of psychoanalysis in the routine practice in psychiatric institutions. In the first part the various levels of effectiveness of psychoanalysis are described. At the lowest level psychoanalysis, like any other science provides specific diagnostic and therapeutic tools. The next higher level is defined by a metatheoretical approach allowing an understanding of the dynamics of the interpersonal relationships in therapy. The next level addresses supervision that helps to analyze the therapeutic relationships in a systematic way. Last but not least psychoanalysis is not only concerned with therapy in the institution but also with therapy of the institution, meaning that psychoanalysis supports the transformation of the institution itself into a therapeutic milieu. In the second part, the prerequisites necessary for psychoanalysis to become a relevant therapeutic factor in psychiatry are highlighted. A differentiation must be made between organizational prerequisites, such as having enough time to devote to the patient and enough therapists with a psychoanalytic background and conceptual prerequisites, whereby all levels of the hierarchy should be supportive and tolerant towards the psychoanalytical perspective. Other factors include appropriate teaching methods. The papers ends with a plea for more psychoanalysts to become involved in psychiatry.
