Zusammenfassung
Gefäßverschlüsse sind das klinische Leitbild der Sichelzellenanämie und führen in der Urologie zu Papillennekrosen, Niereninfarkten oder einem Priapismus. Ein segmentaler Hodeninfarkt bei Patienten mit Sichelzellenanämie ist ein sehr seltenes Ereignis und führte als Tumor bisher fast immer zur Orchiektomie. Wir präsentieren einen 25-jährigen Mann mit seit 3 Tagen zunehmenden Hodenschmerzen rechts und bekannter Sichelzellenanämie. Sonographisch wies der rechte Hoden eine inhomogene, überwiegend hypoechogene Raumforderung mit hyperechogenem Randsaum und keinerlei Perfusion auf. Es wurde eine partielle Orchiektomie durchgeführt mit kompletter Enukleation der Raumforderung, welche histologisch ein hämorrhagischnekrotisches Hodengewebe ohne Anhalt für Malignität erbrachte.
Abstract
Vascular occlusions are the clinical indicators of sickle cell disease and in urology they can lead to papillary necrosis, renal infarction or priapism. Segmental testicular infarction in patients with sickle cell disease is a rare event and only a few cases have been reported. We present a 25-year-old man with right testicular pain increasing over 3 days and sickle cell disease. Ultrasound of the right scrotum presented an inhomogeneous, mainly hypoechegenic mass with a hyperechogenic margin and no sign of blood flow. A partial orchiectomy was performed with total enucleation of the lesion, which was histologically diagnosed as benign hemorrhagic necrotic testicular tissue.
