Fieber, Kopfschmerzen und Apathie bei einer 4½-jährigen Patientin mit homozygoter Sichelzellkrankheit

Zusammenfassung

Wir beschreiben den Fall eines 4½-jährigen Mädchens mit funktioneller Asplenie bei homozygoter Sichelzellkrankheit (SCD-S/S) und Pneumokokkenmeningitis (Pneumokokken Serotyp 23A). Die Infektion trat trotz mehrmalig erfolgter Pneumokokkenimpfung mit dem Konjugatimpfstoff Prevenar13® auf, die Einnahme einer oralen Penicillinprophylaxe war aufgrund mangelhafter Compliance nicht erfolgt.

Durch in Deutschland zur Verfügung stehende Impfstoffe werden 23 (Pneumovax®) bzw. 13 (Prevenar13®) von über 90 verschiedenen Pneumokokkenserotypen abgedeckt. Nach aktuellen Leitlinien wird für Patienten mit Asplenie eine sequenzielle Impfung mit beiden Impfstoffen empfohlen. Die von der STIKO empfohlenen Impfungen sind, wie der vorliegende Fall exemplarisch zeigt, als alleinige Präventionsmaßnahme nicht ausreichend. Wir diskutieren außerdem die Notwendigkeit einer antibiotischen Prophylaxe sowie anhaltend hoher Wachsamkeit der behandelnden Ärzte trotz leitliniengerechter Impfung von Aspleniepatienten.

Abstract

We describe the case of a 4½-year old girl with functional asplenia due to homozygous sickle-cell disease (SCD-S/S) who developed severe pneumococcal meningitis (pneumococcus serotype 23A). The infection occurred despite repeated immunization with Prevenar13®. Due to poor compliance, the additionally recommended oral prophylaxis with penicillin had not been taken.

Vaccines commonly used in Germany cover 23 (Pneumovax®) or 13 (Prevenar13®) out of over 90 different pneumococcus serotypes, respectively. For patients with asplenia, sequential immunization with both vaccines is recommended. As the case presented here highlights, standard vaccination regimes as recommended by STIKO are not sufficient. Furthermore, administration of oral antibiotic prophylaxis, as well as a constant awareness of attending physicians, are advisable even with correct vaccination of asplenic patients.
