Zusammenfassung
Das Allylamin Terbinafin ist das in Deutschland wohl am häufigsten verordnete systemische Antimykotikum zur Behandlung von Dermato- und Onychomykosen. Zugelassen für Patienten ab 18 Jahren, wird Terbinafin jedoch entsprechend Arzneimittelgesetz als individueller Heilversuch immer mehr auch als Off-Label-Medikament bei Kindern zur Behandlung von Onychomykosen sowie Tinea capitis eingesetzt. Terbinafin weist wenige Arzneimittelinteraktionen auf, weshalb es bei älteren und multimorbiden Patienten meist problemlos eingesetzt werden kann. Dennoch sollte man einige potenzielle Interaktionen von Terbinafin mit bestimmten Arzneistoffen kennen. Das betrifft z. B. Wirkstoffe aus der Gruppe der Antidepressiva/Antipsychotika sowie einige kardiovaskulär wirksame Substanzen. Ausschlaggebend für die Relevanz der Interaktionen ist neben der therapeutischen Breite des Substrates und möglichen alternativen Abbauwegen auch der genetisch determinierte Metabolisierungstyp. Bei Kombination von Terbinafin mit trizyklischen Antidepressiva oder selektiven Serotoninwiederaufnahmehemmern und Serotonin/Noradrenalinwiederaufnahmehemmern ist das klinische Ansprechen bzw. das verstärkte Auftreten von Nebenwirkungen zu monitoren. Problematisch ist die Anwendung von Terbinafin bei gleichzeitiger Behandlung mit Tamoxifen. Die Gabe von starken CYP2D6-Hemmern führt zu einer verminderten Wirksamkeit von Tamoxifen, da einer der wichtigsten aktiven Metaboliten, Endoxifen, nicht in ausreichendem Maß zur Verfügung steht. Deshalb sollte diese Kombination mit Terbinafin vermieden werden. Insgesamt ist die Anzahl der Substanzen, die bei gleichzeitiger Therapie mit Terbinafin klinisch relevante Interaktionen verursachen können, übersichtlich und in der Hautarztpraxis bei entsprechendem Monitoring gut zu handhaben.
Abstract
The allylamine terbinafine is the probably most frequently prescribed systemic antifungal agent in Germany for the treatment of dermatomycoses and onychomycoses. According to the German drug law, terbinafine is approved for patients who are 18 years and older; however, this antifungal agent is increasingly used off-label for treatment of onychomycoses and tinea capitis in children. Terbinafine is associated with only a few interactions with other drugs, which is why terbinafine can generally be used without problems in older and multimorbid patients. Nevertheless, some potential interactions of terbinafine with certain drug substances are known, including substances of the group of antidepressants/antipsychotics and some cardiovascular drugs. Decisive for the relevance of interactions is—along with the therapeutic index of the substrate and the possible alternative degradation pathways—the genetically determined type of metabolism. When combining terbinafine with tricyclic antidepressants or selective serotonin reuptake inhibitors and serotonin/noradrenalin reuptake inhibitors, the clinical response and potential side effects must be monitored. Problematic is the use of terbinafine with simultaneous treatment with tamoxifen. The administration of potent CYP2D6 inhibitors leads to a diminished efficacy of tamoxifen because one of its most important active metabolites—endoxifen—is not sufficiently available. Therefore, combination of tamoxifen and terbinafine should be avoided. In conclusion, the number of substances which are able to cause clinically relevant interactions in case of simultaneously administration with terbinafine is clear and should be manageable in the dermatological office with adequate monitoring.
