Zusammenfassung
Etwa 7 % der Melanome weisen eine BRAF-Mutation außerhalb des Codons 600 auf. Diese Mutationen können BRAF aktivieren, ohne therapeutisch mit einem zugelassenen selektiven BRAF-Inhibitor adressierbar zu sein. Hier wird der Fall eines Patienten mit einem fulminant metastasierenden Melanom vorgestellt, das eine BRAFL597Q-Mutation aufweist. Es kann exemplarisch gezeigt werden, dass dieser Tumor exzellent auf eine Therapie mit dem MEK-Inhibitor Trametinib anspricht. Dies ist ein Beispiel für eine mögliche zielgerichtete Therapie bei einem nicht-V600-BRAF-mutierten Melanom, die zu einem 17-monatigen Überleben des Patienten geführt hat.
Abstract
Approximately 7 % of melanomas have a BRAF mutation beyond codon 600. These mutations can be BRAF activating without being addressable by an approved BRAF inhibitor. The case of a patient with fulminant metastatic melanoma and a BRAFL597Q mutation is presented. It is demonstrated that the tumor shows an excellent response to the MEK inhibitor trametinib. This is an example for possible targeted therapy in a non-V600-mutated melanoma resulting in a 17-month overall survival.
