Zusammenfassung
In der Literatur sind nur wenige Fälle einer kontaktallergischen Gastritis bei Patienten mit Nickelallergie beschrieben. Wir berichten über eine 46 Jahre alte Patientin mit kontaktallergischer Gastritis. Im Rahmen der klinischen Untersuchung fanden sich lichenoide Zahnfleischveränderungen im Bereich von erst vor wenigen Wochen eingebrachten Zahnkronen und einer Brücke. Seit Einsetzen des Zahnersatzes litt die Patientin auch unter gastrointestinalen Beschwerden, v. a. Magenschmerzen. Eine Gastroskopie mit Biopsieentnahme hatte eine eosinophile Gastritis gezeigt. Eine Epikutantestung, die aufgrund der Verdachtsdiagnose einer kontaktallergischen Stomatitis erfolgt war, zeigte Sensibilisierungen gegenüber Natriumthiosulfatoaurat, Palladiumchlorid, Vanadium(III)-chlorid, Mangan(II)-chlorid, Zirkonium(IV)-chlorid, Nickel(II)-sulfat sowie gegenüber Duftstoffmix, Duftstoffmix II und dem darin enthaltenen 4‑(4-Hydroxy-4-methylpentyl)-3-cyclohexen-1-carboxaldehyd. Da der Zahnersatz Gold, Palladium und Zirkoniumoxid enthielt, erfolgte ein Austausch gegen titanbasierte Kronen und -brücken. Kurz nach Austausch des Zahnersatzes kam es ohne weitere Behandlung zu einem kompletten Verschwinden aller Symptome und Beschwerden. Eine Allergie vom Spättyp auf Allergene in Zahnkronen und -brücken ist als Ursache der Beschwerden anzunehmen.
Abstract
Only a few cases of contact allergic gastritis in patients with nickel allergy have been reported in the literature. We report a case of probable contact-allergic gastritis in a 46-year-old woman. Clinical examination revealed lichenoid mucosal lesions of the gums adjacent to a bridge and crowns that had been implanted several weeks previously. Since implantation, the patient suffered from gastrointestinal complaints including stomach pain. Gastroscopy and histological investigation of stomach biopsies showed eosinophilic gastritis. Patch testing done under the diagnosis of contact allergic stomatitis showed positive reactions to gold sodium thiosulphate, manganese (II) chloride, nickel (II) sulphate, palladium chloride, vanadium (III) chloride, zirconium (IV) chloride, and fragrances. The crowns and the bridge contained gold, palladium, and zirconium, hence they were replaced by titan-based dentition. Shortly after replacing the artificial dentition, all gastrointestinal symptoms resolved spontaneously without further treatment. Delayed-type allergy to components in the artificial dentition seem to have caused the gastritis.
