Zusammenfassung
Die Bedeutung von Bakterien ist in den letzten Jahren zunehmend in den Fokus der Behandlung von Patienten mit chronischen Wunden gerückt. Für den Nachweis ist es im klinischen Alltag meist ausreichend, einen bakteriologischen Abstrich von der Wundoberfläche entsprechend der Technik „Essener Kreisel“ zu entnehmen. Bei Patienten mit chronischem Ulcus cruris können in Deutschland derzeit Staphylococcus aureus bei ca. 50 %, Pseudomonas aeruginosa und Enterobakterien bei ca. 30 % und Methicillin-resistenter Staphylococcus aureus bei ca. 10 % der Patienten nachgewiesen werden. Meist handelt es sich bei den Patienten um klinisch unkritische bakterielle Kolonisationen, sodass eine systemische Gabe von Antibiotika nur selten erforderlich ist. Für die Wundreinigung sollten ausschließlich sterile Lösungen wie beispielsweise Ringer- oder physiologische Kochsalzlösung verwendet werden. Bei dem Einsatz von Leitungswasser sollten unbedingt Sterilfilter genutzt werden. Für Patienten mit erhöhtem Infektionsrisiko, kritischer Kolonisation oder lokalen Infektionszeichen stehen aktuell mit Polihexanid oder Octenidin moderne Wundantiseptika zur Verfügung. Der Einsatz von systemischen Antibiotika sollte strikt auf Patienten mit systemischen Infektionszeichen beschränkt bleiben.
Abstract
The importance of bacteria in the treatment of patients with chronic wounds has been a focus of attention in recent years. A bacteriological swab taken from the wound in a Essen rotary technique is usually adequate for the diagnosis. The current detection rate of bacteria in patients with chronic leg ulcers in Germany is about 50 % for Staphylococcus aureus, 30 % for Pseudomonas aeruginosa and Enterobacteriaceae and approximately 10 % for MRSA. Most of these patients have a clinically unimportant bacterial colonization, so that systemic administration of antibiotics usually is not required. For wound cleansing only sterile solutions such as Ringer’s or physiological saline solution should be used. If tap water is used, then the use of sterile filters is strongly recommended. In patients with increased risk of infection, a critical colonization or local signs of infection, modern wound antiseptics using polyhexanide or octenidine are available. The use of systemic antibiotics should be strictly limited to patients with systemic signs of infections.
