Abstract.
The phenotypic classification of pancreatic neoplasms is based on their cellular lineage. Thus, tumors with a ductal, acinar and endocrine phenotype can be distinguished. Most pancreatic neoplasms show a ductal phenotype and can be classified as ductal adenocarcinomas. Less common tumors with a ductal phenotype are the variants of ductal adenocarcinoma, intraductal papillary-mucinous neoplasm (including colloid carcinoma), mucinous cystic neoplasm, medullary carcinoma and other rare tumors.
Ductal adenocarcinomas most likely develop on the basis of ductal proliferative lesions arising in the pancreatic duct system. A recently adopted classification system for these lesions distinguishes between three grades of pancreatic intraepithelial neoplasia (PanIN). Molecular studies revealed that PanIN-2 and PanIN-3 lesions represent a distinct step toward invasive carcinoma.
Zusammenfassung.
Die Tumoren des Pankreas werden histopathologisch linienspezifisch nach ihrem Phänotyp klassifiziert. Dabei werden prinzipiell duktale, azinäre und endokrine Tumoren unterschieden. Die meisten Pankreastumoren sind vom duktalen Phänotyp, und zu ca. 90% handelt es sich um duktale Adenokarzinome. Varianten des duktalen Adenokarzinoms weisen ebenso einen duktalen Phänotyp auf wie die intraduktal papillär-muzinöse Neoplasie (das Kolloidkarzinom eingeschlossen), die muzinös-zystische Neoplasie oder das medulläre Karzinom.
Duktale Adenokarzinome entwickeln sich stufenweise aus proliferierenden Läsionen des Pankreasgangepithels. Kürzlich wurden ein Klassifikationssystem und Kriterien erstellt, anhand deren drei verschiedene Grade der pankreatischen intraepithelialen Neoplasie (PanIN) unterschieden werden können. Durch molekulargenetische Untersuchungen wurde bewiesen, dass die Progression von PanIN-2 nach PanIN-3 einen wichtigen Schritt zur Entwicklung eines invasiven Pankreaskarzinoms darstellt.
