Hakpijn: resultaten van operatieve behandeling

Samenvatting

Pijnklachten aan de dorsale zijde van de hiel bij sporters komen frequent voor. In de literatuur heeft dit pijnsyn-droom verscheidene benamingen. De oorzaak van de klachten ligt in het feit dat er frictie bestaat tussen schoen, calcaneus en achillespees. Röntgenologisch kan er sprake zijn van een benige overgroei van het tuber calcanei (haglundse exostose). Als er geen exostose aanwezig is, spreekt men bij deze klachten van een syndroom van Haglund.

Chirurgische behandeling wordt toegepast wanneer conservatieve therapieën geen resultaten hebben opgeleverd. Tussen 1993 en 2004 werden35 patiënten met haglundse exostose dan wel haglundsyndroom behandeld. De ziektegeschiedenissen werden nagekeken en de resultaten na de operatie(s) werden in een telefonisch interview geëvalueerd.

Uit onze studie blijkt dat de chirurgische behandeling van een haglundse exostose of haglundsyndroom goede resultaten geeft, met relatief weinig complicaties.

Abstract

Posterior heel pain occurs often among the athlete. Several names are used to describe this pain syndrome in the medical literature. The cause of this syndrome is associated with friction between the shoe, the os calcaneus and the Achilles tendon. The posterosuperior edge of the os calcaneus can be enlarged on the x-ray (Haglund exostosis). If this part is not enlarged on the x-ray, then this pain complex is called a Haglund syndrome. Surgery is indicated if conservative therapy was not succesful. Between 1993 and 2004, 35 patients received surgical treatment because of posterior heel pain (Haglund exostosis or Haglund syndrome). Their medical charges were evaluated and the outcome of the surgical treatment was also evaluated by a phone call. Our study showed that surgical treatment of a Haglund exostosis or Haglund syndrome provides good results and only a few complications.
