Aan autisme verwante contactstoornissen: klinisch beeld en classificatie

Samenvatting

Kinderen (en volwassenen) met een pervasieve ontwikkelingsstoornis nao hebben ernstige problemen in de sociale ontwikkeling. Dit artikel beschrijft hoe de kernproblemen, de sociale en communicatieve beperkingen tot uiting komen in verschillende levensfasen. Vervolgens komen bijkomende problemen aan de orde: beperkte en stereotiepe gedrags- en interessepatronen, spraak- en taalproblemen, sensorische problemen, problemen in de verstandelijke ontwikkeling en leren, problemen in de motoriek en angst. Ook besteden we aandacht aan het diagnostisch proces, prevalentie en etiologie. Tot slot volgt de bespreking van belangrijke problemen rond diagnostiek en classificatie. Door het ontbreken van positief geformuleerde en eenduidige diagnostische criteria is in de klinische praktijk vaak onduidelijk wanneer de classificatie pervasieve ontwikkelingsstoornis nao moet worden gebruikt.

Abstract

Children and adults with a pervasive developmental disorder not otherwise specified (pdd-nos) show a deviant social development. This article describes the social and communicative problems in the different stages of life and additional problems, such as stereotyped patterns of behaviour and interests, problems in speech and language, sensory problems, problems in cognitive development and learning, motor problems and anxiety. The article provides a short description of the diagnostic process, prevalence and aetiology of the disorder and ends with a discussion of diagnostic and classificatory problems. Since the official classification systems do not describe positive and unambiguous diagnostic criteria, it is often unclear for clinicians when to assign the classification of pdd-nos.
