Résumé
Les auteurs rapportent deux cas de fistule bilio-digestive spontanée, compliquée d’abcès sous-phrénique, l’une cholécystoduodénale secondaire à une cholécystite aiguë lithiasique chez un homme de 70 ans et l’autre duodéno-vésiculaire secondaire à un ulcère du bulbe chez une femme de 59 ans. Le diagnostic fut assuré dans les deux cas par opacification directe de la fistule puis de l’abcès après cathétérisme endoscopique de l’orifice fistuleux duodénal. Les fistules bilio-digestives spontanées, inconstamment suspectées avec les méthodes radiologiques conventionnelles sur l’existence d’une aérobilie et/ou d’un reflux de baryte dans l’arbre biliaire, doivent, à notre sens, être reconnues en phase pré-opératoire grâce à cette méthode de choix.
Summary
The authors report two cases of spontaneous bilio-digestive fistula complicated by subphrenic abscess: one case of cholecystoduodenal fistula secondary to acute lithiasic cholecystitis in a 70-year old man, and another case of duodeno-vesicular fistula secondary to bulbar ulcer in a 59-year old woman. In both cases, diagnosis has been made by direct opacification of the fistula, then the abcess, following endoscopic catheterization of the fistulous opening into the duodenum. To our opinion, spontaneous bilio-digestive fistula, not always detected by standard radiologic techniques on the basis of aerobilia and/or barium reflux into the biliary tree, should be pre-operatively recognized by this choice procedure.
