Résumé
La multiplicité même des techniques témoigne des difficultés d’exploration du reflux gastro-œsophagien de l’enfant, difficultés liées au caractère parcellaire des renseignements fournis par les différents moyens d’exploration. La comparaison de l’échographie à l’endoscopie et à la pHmétrie permet en précisant la Physiopathologie du reflux, de lui restituer son unité et de cerner l’apport des différentes techniques. Dans cette étude ont été abordées essentiellement les complications œsophagiennes du reflux gastroœsophagien du nourrisson et du jeune enfant. Il est apparu qu’elles sont liées à un déséquilibre anatomique dû à la rencontre d’un cardia court et d’un pylore obstructif. Ce déséquilibre entraîne à son tour un dysfonctionnement permanent géné rateur de complications œsophagiennes. En contribution à l’étude de ce déséquilibre, l’endoscopie précise la morphologie du cardia et l’état de la muqueuse œsophagienne, la pHmétrie révèle le degré de dysfonctionnement et l’échographie a une double approche morphologique (œsophage intra-abdominal et pylore) et fonctionnelle.
Summary
The multiplicity of the techniques in itself is an indication of the difficulties encountered in the exploration of gastro-œsophageal reflux in children. These difficulties are related to the fragmentary nature of the data provided by the various individual exploration techniques. The comparison of echography, endoscopy and pHmetry, while enabling to clarify the physiopathology of the reflux, allows to restitute its unity and to delimit what the various techniques can contribute. In this study, the œsophageal complications of gastro-œsophageal reflux of babies and small children have been addressed. It was seen that they are related to an anatomical imbalance due to the conjunction of a short cardia and an obstructive pylorus. This disequilibrium in turn leads to a permanent dysfunction which generates œsophageal complications. In the study of this disequilibrium, endoscopy contributes the morphology of the cardia and the state of the œsophageal mucosa, the pHmetry reveals the degree of dysfunction while echography has a double approach which is both morphological (intra-abdominal œsophagus and pylorus) and functional.
Resumen
La misma multiplicidad de las técnicas testified las dificultades en ta exploration del reflujo gastro-esofágico del niño, dificultades ligadas al carácter particular de las informaciones dados por los diferentes medios de exploratión. La comparación de la ecografía, la endoscopia y la pHmetria permite precisar la fisiopatologia del reflujo, de restitulrle su unidad y de delimitar el aporte de las diferentes técnicas. En este estudio han sido abordadas esencialmente las complicaciones esofágicas del reflujo gastro-esofágico del lactante y del nino. Aparece que ellas están ligadas a un desequilibrio anatómico debido a la presencia de un cardias corto y un píloro obstruído. Este desequilibrio trae a su vez un disfuncionamiento permanente generador de complicaciones esofágicas. En contribution al estudio de este desequilibrio, la endoscopia precisa la morfología del cardias y el estado de la mucosa esofágica, la pHmetría revela el grado de disfuncionamiento y la ecografía tiene un doble enfoque morfológico (esófago intra-abdominal y piloro) y funcional.
Zusammenfassung
Die Vielzahl an Untersuchungstechniken spiegelt die Schwierigkeit der Diagnostik der Refluxösophagitis beim Kind wieder. Diese sind durch die bruchstöckhaften Aussagen der verschiedenen Untersuchungstechniken bedingt. Der Vergleich der Sonographie mit der Endoskopie und der pH-Metrie erlaubt die Physiopathologie des Refluxes zu bestimmen, ihn als eine Einheit zu sehen und den Beitrag der verschiedenen Untersuchungstechniken einzugrenzen. In dieser Studie werden vor allem die Komplikationen der Refluxösophagitis beim Säugling und Kleinkind erörtert. Es ist offensichtlich, daβ diese bedingt sind durch ein anatomisches Miβverhältnis zwischen einer kurzen Kardia und einer Pylorusobstruktion. Dieses Miβverhältnis führt seinerseits zu einer permanenten Dysfunktion mit ösophaguskomplikationen. Zu der Diagnostik dieser Veränderungen trägt die Endoskopie bei, Morphologie der Kardia und der Ösophagusschleimhaut darzustellen; die pH-Metrie deckt den Grad des Dysfunktion auf und die Sonographie erlaubt den morphologischen (intraabdomineller Ösophagus und Pylorus) und funktionellen Zugang.
Riassunto
L’utilizzazione di numerose tecniche diagnostiche testimonia la difficoltànello studio del reflusso gastro-esofageo nel bambino, difficoltàlegate al carattere limitato delle informazioni formte dalle differenti metodiche di indagine. Il confronte dell’ecografia con l’endoscopia e la pH-metria permette di studiare con precisione i meccanismi fisiopatologici del reflusso e di precisare l’apporto delle differenti teeniche nello studio di questa entitànosologica. In questo lavoro sono descritte essenzialmente le complicazioni esofagee del reflusso gastro-esofageo del luttante e del bambino. Queste son legate ad un alterato equilibrio anatomico secondario alla presenza di un cardias corto e di un piloro stenotico. Questo squilibrio porta a sua volta ad un malfunzionamento permanente che èalla base delle complicazioni esofagee. Nello studio di queste alterazioni, l’endoscopia precisa la morfologia del cardias e lo stato della mucosa esofagea, la pH-metria rivela il grado di severitàdell’anomalia e l’ecografia permette un doppio approccio morfologico (a livello dell’esofago intra-addominale e del piloro) e funzionale.
