Riassunto

L'A. dà la definizione della resistenza insulinica cronica e ne riassume le caratteristiche cliniche. Sebbene nella maggior parte dei pazienti con insulino-resistenza cronica possa essere dimostrato l'aumento di anticorpi anti-insulina, non sempre esiste una buona correlazione tra gravità dell'insulino-resistenza e titolo di anticorpi anti-insulina. L'A. descrive diversi casi di pazienti insulino-resistenti che potrebbero rappresentare esempi di insensibilità tissutale periferica all'insulina, ed infine passa in rassegna varie forme di trattamento dell'insulino-resistenza cronica.

L'A. donne une définition de la résistance insulinique cronique et fait un résumé des caractéristiques cliniques. Dans la majorité des sujets avec insulino-résistance cronique on peut démontrer l'augmentation des anticorps anti-insuliniques, tandis que n'existe pas toujours une bonne corrélation entre la gravité de l'insulino-résistance et le titre des anticorps anti-insuliniques. L'A. décrit plusieures cas de sujets insulino-résistantes qui pourraient représenter des exemples d'insensibilité tissulaire périphérique à l'insuline et discute sur la possibilité de différents traitements de l'insulino-résistance chronique.

Resumen

El A. nos define la resistencia crónica a la insulina y nos resume sus características clínicas. Aunque en la mayor parte de los enfermos insulinorresistentes crónicos se pueda demostrar el aumento de anticuerpos antinsulina, no siempre existe una buena correlación entre la gravedad de la insulinorresistencia y el porcentaje de anticuerpos antinsulina. El A. describe diversos casos de enfermos insulinorresistentes que podrían constituir ejemplos de insensibilidad hística periférica a la insulina y finalmente expone varias formas de tratamiento de la insulinorresistencia crónica.

Zusammenfassung

Der A. liefert die Definition der chronischen Insulin-Resistenz und fasst die klinischen Merkmale zusammen. Obwohl man beim Grossteil der Patienten mit chronischer Insulin-Resistenz die Zunahme der Insulin-Antikoerper nachweisen kann, besteht nicht immer eine gute Korrelation zwischen Schweregrad der Insulin-Resistenz und Insulinantikoerper-Titer. Der A. beschreibt einige Faelle von insulin-resistenten Patienten, die das Beispiel einer peripheren Gewebeunempfindlichkeit gegenueber dem Insulin darstellen koennten und bespricht zuletzt verschiedene Behandlungsformen der chronischen Insulin-Resistenz.

Summary

Chronic insulin resistance has been defined and its clinical characteristics summarized. Although increased insulin antibodies can be demonstrated in most patients with chronic insulin resistance, there is not always a good correlation between the severity of the insulin resistance and the titer of insulin antibodies. Several insulin resistant patients have been described who appear to represent examples of peripheral tissue unresponsiveness to insulin. Various forms of treatment of chronic insulin resistance have been reviewed.
