Das Pankreas anulare und seine chirurgische Behandlung

Zusammenfassung

Erfahrungen mit der chirurgischen Behandlung des Pankreas anulare sind in der Hand des einzelnen Operateurs relativ gering. Der Befund wird präoperativ häufig und gelegentlich sogar während des Eingriffs verkannt. Da sich die Mitteilungen über operierte Fälle allein während der letzten 10 Jahre fast verdoppelt haben-es hantelte sich allerdings in der Mehrzahl um Einzelbeobachtungen-scheint das Ringpankreas doch etwas häufiger vorzukommen als bisher angenommen wurde. Eine Schilderung des Krankheitsbildes erschien daher gerechtfertigt und stützt sich auf die eigenen Erfahrungen bei zehn Kranken (ein Neugeborenes und neun Erwachsene). Außerdem wurde das Weltschrifttum ausgewertet und früheren Sammelstatistiken 159 Fälle der Jahre 1957 bis 1967 hinzugefügt. Eine kritische Diskussion der verschiedenen Operationsverfahren führte zu der Erkenntnis, daß eine direkte Ringdurchtrennung risikoreich, hingegen die Duodenojejunostomie im Neugeborenenalter und bie Erwachsenen ohne komplizierendes Ulcus als Methode der Wahl anzusehen ist. Liegt gleichzeitig ein Ulcus vor, das häufig in das Ringpankreas penetriert, so wird man eine gastroduodenale Resektion vornehmen. Die Prognose ist bei Erwachsenen sehr günstig, bei Neugeborenen weitgehend vom Vorhandensein und Ausmaß weiterer Mißbildungen abhängig.
