Zusammenfassung
Um die Veränderungen der vor und nach dem Herzinfarkteingesetzten Medikation zu beurteilen, wurden die im Zeitraum 1985–1988 im Herzinfarktregister Augsburg registrierten und überlebenden Herzinfarktfälle im Alter von 25–74 Jahren nach Geschlecht, Anamnese und Medikation ausgewertet. Bei den 1546 Patienten (1181 Männer, 365 Frauen) lag die Behandlungshäufigkeit mit fast allen Substanzgruppen bei Reinfarktpatienten höher als bei Erstinfarktpatienten. Vor dem Infarkt, jedoch nicht danach, erhielten Frauen signifikant mehr Arzneimittel als Männer und wurden häufiger mit Diuretika (30%, Männer 18%), Antihypertensiva (15%, Männer 8%) und Herzglykosiden (20%, Männer 14%) behandelt. Die am häufigsten eingesetzten Arzneigruppen sind Nitrate (vor AMI: 30%, nach AMI: 80%) und Kalzium Antagonisten (vor AMI: 26% nach AMI: 61%). In den vier Jahren zeigte sich ein signifikanter Anstieg der Reinfarktpatienten, die vor ihrem Reinfarkt mit Thrombozytenaggregationshemmern behandelt wurden (1985: 9%, 1988: 32%), ebenso bei Männern mit Erstinfarkt bei der Krankenhausentlassung (1985: 38%; 1988: 64%). Während des vierjährigen Studienzeitraumes wurde ein signifikanter Abfall bei der Reinfarktrate der Männer beobachtet. Insgesamt akezptiert die klinische Praxis relativ rasch die aus klinischen Studien gewonnenen Erkenntnisse, was sich bis auf die Substanzebene nachvollziehen lässt.
Summary
To assess the changes in medications prescribed before and after acute myocardial infarction (AMI), all surviving patients aged 25–74 years registered from 1985–1988 in the Coronary Event Register Augsburg were evaluated by sex, medical history, and drug use before and after the event. For the 1546 hospitalized patients (1181 men, 365 women) utilization of all drug groups is higher for patients with reinfarction than for patients with first-ever AMI. Before AMI, but not on hospital discharge, women received significantly more medications than men and were more frequently treated with diuretics (30%, men 18%), antihypertensive drugs (15%, men 8%) and cardiac glycosides (20%; men 14%). The most frequently used drug groups are nitrates (before AMI: 30%; after AMI: 80%) and calcium antagonists (before AMI: 26%; after AMI 61%). The four years show a significant increase in patients with reinfarction who were treated with platelet aggregation inhibitors (1985: 9%; 1988: 32%), as well as an increase in AMI patients released from hospital with this medication (1985: 38%; 1988: 64%). A concomitant significant decrease in reinfarction rates for men is found in the course of the four years under study. The results indicate a rapid assimilation of the results of clinical trials in practice down to the substance level.
Résumé
Afin d'évaluer les changements dans la prescription de médicaments avant et après un infarctus du myocarde (IM), les cas enregistrés au registre des IM d'Augsburg entre 1985 et 1988 furent analysés selon le sexe, l'anamnèse et la médication. Ces données concernaient 1546 patients (1181 hommes et 365 femmes) ayant survécu à l'infarctus, âgés de 25 à 74 ans. Les cas de récidive d'infarctus avaient tendance à recevoir un traitement médicamenteux plus fourni que les cas inauguraux, ceci pour presque toutes les classes thérapeutiques. Avant l'épisode, mais pas après, les femmes recevaient significativement plus de médicaments que les hommes, notamment de diurétiques (30%, hommes 18%), antihypertenseurs (15%, hommes 8%) et digitaliques (20%, hommes 14%). Les substances les plus fréquemment prescrites furent les dérivés nitrés (avant IM: 30%, après: 80%) et les anti-calciques (avant IM: 26%, après: 61%). Pendant ces quatre années, on observait une partsignificativement croissante de patients traites par anti-aggrégants plaquettaires parmi ceux admis à cause d'une récidive d'IM (1985: 9%, 1988: 33%), ainsi que parmi les hommes quittant l'hôpital après un IM inaugural (1985: 38%, 1988: 64%). Pendant cette période, une diminution significative des cas inauguraux d'IM chez les hommes de 55 à 64 ans, ainsi que des cas de récidive d'IM parmi les hommes de 65 à 74 ans fut observée. Dans l'ensemble, ces résultats suggèrent que la pratique clinique accepte et intègre relativement rapidement les découvertes des études cliniques.
