Zusammenfassung
Nach Isolierung des graviden Kaninchen-Uterus aus dem maternen Organismus und Durchblutung durch Spendertiere über eine Aorta-Aorta-Anastomose wurden die Durchblutung und der Sauerstoffverbrauch bei Normothermie ermittelt. Untersucht wurde der Zeitraum zwischen dem 22. und 34. Tag der normalerweise 31tägigen Tragzeit. Die Tragzeitverlängerung wurde durch Progesterongaben erzielt.
Bei 14 Versuchen überlebten die Feten für die Dauer von 2–5 1/2 Std, in fünf Fällen — viermal nach verlängerten Tragzeiten — kam es bei Versuchsbeginn zum intrauterinen Fruchttod.
Zwischen dem 22. und 30. Tag stiegen bei 36 °C die Durchblutungswerte von 7 auf 15 ml/min/100 g gravider Uterus und die O2-Verbrauchswerte von 1,0 auf 2,1–2,4 ml/min/100 g gravider Uterus. Der am Wurftermin maximale O2-Verbrauch verminderte sich bei verlängerten Tragzeiten auf 1,8–2,0 ml/min/100 g bei gleichzeitigem Rückgang der Uterusdurchblutung auf 10 ml/min/100 g am 32. Tag.
Die Zunahme von Durchblutung und Sauerstoffverbrauch wird in erster Linie auf einen ansteigenden Stoffwechsel von Feten und Placenten zwischen dem 22. und 30. Tag zurückgeführt. Da bei verlängerten Tragzeiten die Durchblutungs- und O2-Verbrauchswerte niedriger lagen als am 30. Tag, und auffallend häufig die Feten während der Anaesthesie abgestorben waren, wird angenommen, daß bei prolongierten Graviditäten eine placentare O2-Diffusionsstörung (placentare Dysfunktion) und eine uterine Minderdurchblutung den intrauterinen Fruchttod verursachen.
Summary
Pregnant rabbit uteri were isolated and the circulation maintained by donor-animals via an aorta-aorta-anastomosis. Uterine blood-flow and oxygen-consumption were then determined under normothermic conditions for the time from the 22nd to 34th day of pregnancy. The prolongation of the gestation period from normally 31 to 34 days was achieved by progesterone application.
The fetal survival time in 14 test animals ranged from 2–5 1/2 hours, fetal death occured in 5 cases, 4 of which were over term.
Between the 22nd and 30th day of pregnancy the uterine blood flow increased from 7 to 15 ml/min/100 g pregnant uterus and the O2-consumption from 1,0 to 2,1–2,4 ml/min/100 g pregnant uterus (measured at 36 °C). The oxygen consumption, which was found highest at term, was shown to decrease with prolongation of the gestation period to 1,8–2,0 ml/min/100 g, simultaneously the uterus blood flow decreased to values about 10 ml/min/100 g on the 32nd day of gestation. The increase in blood flow and O2-consumption was thought to be due to increased fetal and placental metabolism between the 22nd and 30th day of gestation. Since uterine blood flow and O2-consumption were found to decrease after term and fetal deaths were seen frequently in those cases it was assumed that a placental O2-diffusion disturbance (placental dysfunction) as well as a decrease of uterine blood flow is a factor for the increased fetal death rate in cases with prolonged gestation periods.
