Summary
In the period from 1967 to November 1981, 502 patients with cervical spine injuries were treated surgically using anterior interbody fusion. In 282 patients the Cloward method was used and in 220 the Smith-Robinson technique. Although anterior fusion has been performed at all levels of the cervical spine we have excluded from our analysis cases of fusion of C1/C2 as we consider these constitute a separate problem. The series includes patients with complete neurological sparing as well as all degrees of severity of neurological damage. Neurological improvement was observed in 253 cases and in 72 patients with normal spinal cord function prior to surgery no deterioration occurred. Satisfactory radiological appearance with restitution of the contour of the vertebral canal and sound incorporation of the bone graft was obtained in 70% of cases, whilst failure occurred in 5%.
We believe that surgical fusion should be recommended in most cases of cervical spine injury.
Résumé
De 1967 à Novembre 1981, 502 blessés présentant une lésion traumatique du rachis cervical ont été traités chirurgicalement par arthrodèse vertébrale antérieure: 282 par la technique de Cloward et 220 par celle de Smith-Robinson. Bien que cette intervention ait été réalisée à tous les niveaux, on a exclu de cette étude les cas d'arthrodèse C1–C2 qui paraissent constituer un problème particulier. La série comporte aussi bien les sujets indemne de toute atteinte neurologique que ceux qui présentaient des lésions neurologiques, quel qu'en soit le degré de gravité. Une amélioration neurologique a été observée dans 253 cas et chez les 72 patients dont la fonction médullaire était normale avant l'intervention il n'y a eu aucune aggravation. Une image radiologique satisfaisante avec reconstitution des contours du canal vertébral et une fusion solide du greffon ont été obtenues dans 70% des cas. Il n'y a eu que 5% d'échecs.
Les auteurs estiment que l'arthrodèse corporéale est indiquée dans la plupart des cas de traumatisme du rachis cervical.
