Zusammenfassung
Hintergrund
Chemodenervierung mittels Botulinumtoxin zur ästhetischen Faltenreduktion der Stirn ist einer der weltweit am häufigsten durchgeführten Eingriffe in der ästhetischen Chirurgie. Die Sicherheit und Effektivität ist in vielfältigen Studien bewiesen. Es gibt bisher jedoch nur wenige Untersuchungen, die das Behandlungsergebnis einer Chemodenervierung der Glabella und des M. frontalis objektiv vermessen und Empfehlungen zur Optimierung der Behandlung für verschiedene Patientengruppen geben.
Material und Methoden
Nach Darlegung der relevanten muskulären Anatomie der Glabella und der Stirn werden die üblichen Schemata zur Botulinumtoxinbehandlung der Zornesfalte und der horizontalen Stirnfalten vorgestellt. Anhand von 3D-Oberflächenscans der Gesichter von jeweils zehn Probandinnen vor und nach Botulinumtoxinbehandlung von entweder nur der Glabella oder von Glabella und M. Frontalis wurde die Veränderung der genauen Brauenposition vermessen.
Ergebnisse
Bereits bei der alleinigen Behandlung der Glabella zeigt sich ein Trend zum Absinken der medialen Braue, was sich bei einer Mitbehandlung des M. frontalis verstärkt und nach lateral fortsetzt. Dieser Effekt nimmt mit zunehmendem Alter zu.
Schlussfolgerung
Vor allem bei Patienten mit (kompensierter) Brauenptose ist vor einer Botulinumtoxinbehandlung die genaue Evaluation der Stirn, der Brauen und des Anteils der muskulären Stabilisierung ihrer Position notwendig, um die optimale Anpassung der Chemodenervierung an die individuellen Voraussetzungen der Patienten zu erreichen.
Abstract
Background
Chemodenervation with botulinum toxin for esthetic reduction of wrinkles of the forehead is one of the most commonly performed interventions in esthetic surgery worldwide. The safety and effectiveness has been proven in a multitude of studies; however, only a few studies have objectively measured the results of treatment by chemodenervation of the glabella and the occipitofrontal muscle and given recommendations for optimization of treatment in the various patient groups.
Material and methods
After describing the relevant muscular anatomy of the glabella and the forehead, the routine regimens for botulinum toxin treatment of frown wrinkles and horizontal forehead wrinkles are presented. The alterations in the exact eyebrow positions in ten female subjects before and after botulinum toxin treatment of either the glabella alone or the glabella and occipitofrontal muscle were measured using 3D surface scans of the face.
Results
Even after treatment of the glabella alone there was a trend to sagging of the medial eyebrow which became stronger and laterally extended after inclusion of the occipitofrontal muscle. This effect increased with the age of the subject.
Conclusion
Before treatment with botulinum toxin an exact evaluation of the position of the forehead, eyebrows and the proportion of the muscular stabilization is necessary in order to achieve an optimal adjustment of the chemodenervation to the individual prerequisites of patients, particularly in patients with (compensated) brow ptosis.
