Zusammenfassung

Die gastrointestinalen Lymphome gehören zu der Gruppe der extranodalen Lymphome und bestehen selbst aus vielen heterogenen Subtypen. Häufigster Vertreter ist das Magenlymphom, während intestinale Lymphome eher selten sind. In den letzten Jahren konnten durch verbesserte Diagnostik und Therapie – untersucht in verschiedenen prospektiven Studien – gesicherte Standards etabliert werden. So ist z. B. die primäre Operation nicht mehr die Therapie der Wahl, sondern sollte nur noch bei Notfällen wie Perforation und konventionell nicht beherrschbaren Blutungen durchgeführt werden. Stadium- und histologieabhängige Therapie mit konservativer Organ erhaltender Behandlung zeigte im Vergleich zur Operation vergleichbar gutes rezidivfreies Überleben und Gesamtüberleben. Beim primären Lymphom des Darms bedarf es jedoch weiterer Studien, um gesicherte Empfehlungen zur Behandlung aussprechen zu können.

Abstract

Primary gastrointestinal lymphoma (PGL) belongs to the group of extranodal lymphomas and represents a heterogeneous group of NHL subtypes. The stomach is the most commonly affected site, while intestinal lymphomas are rare. Due to the improvements in diagnostic and therapeutic approaches established in various prospective studies, it has become possible to define novel therapeutic standards. Surgery can be omitted from the treatment of PGL and should only be carried out when perforation, hemorrhage, or obstruction occur, which cannot be managed by conservative treatment. Stage and histology dependent conservative organ-preserving approaches are not inferior to the surgical approach in time to treatment failure and overall survival. In the case of intestinal lymphomas, further studies are needed to define therapeutic recommendations.
