Zusammenfassung
Problemstellung
Der positive Effekt der Laserkoagulation bei diabetischem Makulaödem ist seit der Early Treatment Diabetic Retinopathy Study bekannt. Inwieweit auch eine Vitrektomie als effiziente Behandlung eingesetzt werden kann ist Gegenstand dieser Studie.
Methode und/oder Patienten
In dieser prospektiven Studie wurde anhand der Bestimmung der Netzhautdicke mittels Okulärer Kohärenztomographie (OCT) sowie des bestkorrigierten Visus untersucht, inwieweit die Laserkoagulation im Vergleich mit der Vitrektomie bei diabetischem Makulaödem zu einer Reduktion der Netzhautdicke und einer Visusverbesserung führt. Insgesamt 18 Patienten mit klinisch signifikantem diabetischem Makulaödem wurden eingeschlossen.
Ergebnisse
Die durchschnittliche Netzhautdicke betrug in der Lasergruppe, in die 11 Patienten eingeschlossen wurden, präoperativ im Durchschnitt 378,6 ± 119,6 υm und zeigte nach 3 Monaten eine Reduktion auf 305,1 ± 43,6 μm und ist damit statistisch signifikant (p = 0,047). Auch 6 Monate nach Behandlung zeigte sich eine weitere Abnahme der Netzhautdicke auf 297 ± 44,11 μm (p = 0,055).
In der Vitrektomiegruppe ergaben sich präoperativ zu-nächst Netzhautdickenwerte von 294,9 ± 82,1 μm, nach 3 Monaten 306,1 ± 83,9 μm (p = 0,403) und nach 6 Monaten 284,2 ± 27,5 μm (p = 0,278). Hinsichtlich des Visusverlaufs ergab sich in der Lasergruppe keine statistisch signifikante Änderung. In der Vitrektomiegruppe konnte mit einer Zunahme von 2,6 ± 1,7 Zeilen eine deutliche Visusverbesserung zwischen präoperativem Visus und nach sechs Monaten festgestellt werden.
Zusammenfassung
Wie unsere Studie bisher gezeigt hat, ist sowohl die Vitrektomie als auch die Laserkoagulation in der Behandlung des klinisch signifikanten diabetischen Makulaödems eine effektive Therapie, um das Ödem zu reduzieren. Eine Verbesserung der Sehschärfe zeigte sich vor allem nach der Vitrektomie. Zusammenfassend sind diese Ergebnisse ermutigend, müssen aber durch größere Patientenzahlen und längere Beobachtungszeiträume untermauert werden.
Summary
Purpose
The efficacy of laser photocoagulation in treating diabetic macular edema is known since the Early Treatment Diabetic Retinopathy Study. The aim of our study was to investigate the effect of vitrectomy on diabetic macular edema in comparison with laser photocoagulation.
Methods and Patients
Main outcome criteria in this prospective investigation were best-corrected visual acuity and retinal thickness measured by Optical Coherence Tomography (OCT) to assess the effect of laser photocoagulation compared to vitrectomy on diabetic macular edema. Eighteen patients with clinically significant diabetic macula edema were included.
Results
The mean retinal thickness in the photocoagulation group consisting of 11 patients was 378.6 ± 119.6 μm preoperatively and showed a significant reduction to 305.1 ± 43.6 μm after 3 months (p = 0.047) and 297 ± 44.11 μm (p = 0.055) after 6 months. In the vitrectomy group the mean retinal thickness measured preoperatively was 294.9 ± 82.1 μm, 306.1 ± 83.9 μm (p = 0.403) after 3 months and 284.2 ± 27.5 μm (p = 0.278) after 6 months. No significant change in visual acuity could be observed in the photocoagulation group, whereas an improvement of 2.6 ±1.7 lines was observed in the vitrectomy group.
Conclusion
The current study indicates that both vitrectomy and photocoagulation are effective in the treatment of clinically significant diabetic macular edema in respect to reducing retinal thickness. In respect to improving the viusal acuity only the vitrectomy group showed a benefit. In sum our results are encouraging, but larger patient numbers and long-term follow up are necessary to confirm these results.
