Abstract
Achtergrond: De zorgstandaard diabetes type 2 van de Nederlandse Diabetes Federatie (NDF) beschrijft de zorginhoudelijke en organisatorische eisen aan diabeteszorg. Hij is functioneel omschreven, waardoor taakherschikking mogelijk wordt. De zorgstandaard diende eerst alleen als basis voor het vormgeven van het zorgprogramma, maar met de komst van integrale bekostiging is hij ook een inkoopinstrument geworden. Onderzocht is of de zorgstandaard zorggroepen voldoende houvast biedt bij het vormgeven van de diabeteszorg en bij het zorginkoopproces. Methoden:Het onderzoek omvatte een schriftelijke vragenlijst onder alle zorggroepen (N=89) in Nederland (respons 51%) en tien diepte-interviews. Resultaten: Zorggroepen gebruiken de NDF zorgstandaard zowel bij het opstellen van het zorgprogramma diabetes als bij het zorginkoopproces. Ook maken zorggroepen gebruik van de functionele omschrijving in de vorm van taakherschikking en vindt samenwerking tussen de kerndisciplines plaats. Maar de beoogde structurele samenwerkingsverbanden met overige zorgverleners komen nog weinig tot stand en de centrale rol van de patiënt is nog onvoldoende vorm gegeven. Bovendien missen zorggroepen in de zorgstandaard de mogelijkheid om zorg op maat te kunnen leveren. Conclusie: De zorgstandaard biedt voor zorggroepen voldoende houvast. Wel dient de bruikbaarheid van de zorgstandaard continu geëvalueerd te worden vanwege de tweeledige rol die hij vervult en vanwege de taakherschikking die mogelijk is geworden door de functionele omschrijving. Daarnaast zijn aanpassingen op het gebied van de diversiteit van patiëntgroepen en de centrale rol van de patiënt wenselijk.
Abstract
Does the Diabetes Health Care Standard offer Care Groups enough guidance?
Background : The Diabetes Health Care Standard of the Dutch Diabetes Federation describes the content and organizational requirements of diabetes care. The health care standard is defined functionally, allowing task reallocation.
Initially, the care standard functioned as basis for the diabetes care program. With the introduction of bundled payment for diabetes, the health care standard has become a purchasing instrument as well. This led to health care providers (especially GPs) organizing themselves into care groups. These care groups negotiate with health insurers about the purchasing of standard diabetes care and with other health care providers on the provision of care. The aim of this study was to determine whether the health care standard provides care groups enough guidance in shaping the diabetes care and in the health care purchasing process.
Methods: The study consisted of two phases: 1) written questionnaire distributed to all 89 Dutch care groups 2) 10 in-depth interviews. Results: The response was 51%.
