Summary
Somatoform disorders play an important role in medicine though the diagnostic category itself has been criticised. This antagonism is manifested in psychiatric routine by little attention on somatoform disorders and somatisation. Possible reasons should be discussed in this paper. Therefore we view on the problem of missing specifity of bodily symptoms, the meaning of severity of global functioning of these patients as well as the problem of diagnosis, comorbidity and economy in the process of diagnosis. The ability of the paper is to draw further attention in routinely diagnoses of somatoform disorders.
Zusammenfassung
Somatoforme Störungen spielen im Alltag der Medizin eine bedeutsame Rolle, wenngleich die diagnostische Entität selbst kritisiert wird. In der psychiatrischen Praxis äußert sich dieser Widerspruch durch eine nur geringe Aufmerksamkeit für dieses Störungsbild. Die möglichen Ursachen dafür sollen in diesem Beitrag diskutiert werden und kommen in Form des Problems der fehlenden Spezifität körperlicher Symptome, der Bedeutung des Schweregrads der Beeinträchtigung, sowie in Form des Problems der Differentialdiagnostik und der Ökonomie in der praktischen Diagnostik zu Sprache. Schließlich möchte dieser Beitrag dem psychiatrischen Praktiker die diagnostische Thematik und die derzeit laufende Diskussion nahebringen.
