Remote monitoring of a high-risk patient with Brugada syndrome: association of different arrhythmic manifestations

Abstract

We report on the remote arrhythmia monitoring of a 34-year-old man with highly symptomatic Brugada syndrome, who initially presented with syncope, paroxysmal atrial fibrillation, and spontaneous coved-type electrocardiogram. The patient received a dual-chamber implantable cardioverter-defibrillator (ICD) with Home Monitoring™ facilities and experienced recurrent ICD shocks for spontaneous ventricular fibrillation (VF) episodes during the first year after ICD implantation. Remote monitoring revealed an increased burden of premature ventricular complexes and atrial arrhythmias each time VF spontaneously occurred. Atrial and ventricular arrhythmias were effectively suppressed by low-dose quinidine without severe side effects.

Zusammenfassung

Wir berichten über ein telemetrisches Arrhythmie-Monitoring bei einem 34-jährigen männlichen Patienten mit hoch-symptomatischem Brugada-Syndrom, der sich initial mit Synkope, paroxysmalem Vorhofflimmern und spontanem coved-type Elektrokardiogramm vorstellte. Der Patient erhielt einen Zweikammer-ICD mit der Möglichkeit zu telemetrischem Monitoring und erlitt zahlreiche ICD-Schocks wegen spontanem Kammerflimmern im ersten Jahr nach ICD-Implantation. Telemetrisches Monitoring wies ein gehäuftes Auftreten von ventrikulären Extrasystolen und atrialen Arrhythmien vor jeder Episode von Kammerflimmern nach. Atriale und ventrikuläre Arrhythmien konnten mittels niedrig-dosiertem Chinidin effektiv unterdrückt werden ohne ernste Nebenwirkungen.
