Zusammenfassung
Vorhofflimmern (VHF) ist die häufigste Rhythmusstörungen im Erwachsenenalter mit einer Häufigkeit von 5–8% bei 70-jährigen und etwa 10% bei Patienten (Pt), die älter als 80 Jahre alt sind. Für die Intensiv- und Notfallmedizin ist vor allem das neu aufgetretene oder tachykarde (Kammerfrequenz > 100/min) VHF von Bedeutung. Die Akuttherapie umfasst elektrische oder medikamentöse Kardioversion, Frequenzkontrolle bei permanentem VHF und adäquate Antikoagulation. In bis zu 25% ist ein cerebraler Insult das Erstsymptom von VHF. Pt mit tachykardem VHF und instabiler Hämodynamik sollten sofort elektrisch kardiovertiert werden, bei permanentem VHF ist eine Frequenzkontrolle indiziert. Für die Langzeittherapie stehen sich Konzepte wie Rhythmus- oder Frequenzkontrolle gegenüber, deren Indikation vor allem von Symptomatik, Typ des VHF, Grunderkrankung und linksventrikulärer (LV) Funktion abhängig ist. Medikamente zur Rhythmuskontrolle sind Antiarrhythmika der Klassen I-III, während für die Frequenzkontrolle Betablocker, Calciumantagonisten oder Digitalis allein oder in Kombination herangezogen werden können. Eine antiarrhythmisch-medikamentöse Therapie („Rhythmuskontrolle“) bietet hinsichtlich einer Lebensverlängerung keine Vorteile gegenüber einer Frequenzkontrolle. Die Antikoagulation ist bei den meisten VHF-Pt notwendig und richtet sich ebenfalls nach VHF-Typ, Grundkrankheit und LV-Funktion.
Abstract
Atrial fibrillation (AF) is the most common arrhythmia in adults with an incidence of 5–8% in patients (pts) with an age of 70 years and 10% in pts with an age > 80 years. In emergency and intensive care medicine, there is special interest of newly occurring or fast (heart rate > 100/min) AF. Acute therapy of AF includes electrical or pharmacological cardioversion, rate control and adequate anticoagulation. In 25% of pts with AF, stroke is the first symptom of this arrhythmia. Pts with fast AF and hemodynamic instability are candidates for immediate DC cardioversion, whereas rate control is necessary in pts with permanent AF. For long-term therapy, rhythm or rate control is available. The indication for these strategies depends on symptoms, type of AF, underlying disease or left ventricular (LV) function. Class I-III antiarrhythmic drugs are present for rhythm control, whereas ß-blocking agents, calcium antagonists and/or digitalis are used for rate control. There are no advantages with respect to survival for pts treated with antiarrhythmic drugs (“rhythm control”) compared to pts treated with the “rate control” strategy. Anticoagulation is necessary in the majority of AF pts and its use is dependent on AF type, underlying disease, and LV function.
