Zusammenfassung

Die Geburt eines extrem unreifen Frühgeborenen an der Grenze der Lebensfähigkeit ist eine Herausforderung für alle Beteiligten – Eltern, Ärzte und Pflegende. Es ist mittlerweile gut belegt, dass die augenblickliche Vitalität solcher Kinder im Kreißsaal keine Prognoseabschätzung erlaubt. Im Grenzbereich sollen die Vorstellungen der Eltern maßgeblich sein. Die eigenen Werte der beratenden Ärzte müssen von diesen selbst reflektiert werden, um eine ergebnisoffene, nichtpaternalistische Gesprächsführung zu ermöglichen und eine Einhaltung der mit den Eltern getroffenen Vereinbarungen zu gewährleisten.

Abstract

The birth of an extremely premature infant at the limit of viability poses a challenge to all involved—parents, physicians, and nursing staff. It is now well documented that the immediate vitality of these children in the delivery room does not serve as a basis for assessing prognosis. The parents’ viewpoints should govern decisions in cases of borderline viability. The attending physicians need to reflect on their own opinions to best facilitate an open-minded and non-paternalistic discussion to ensure adherence to the decisions reached with the parents.
