Zusammenfassung
Wir berichten über eine 52-jährige Patientin, die mit Fieber, Schüttelfrost und Myalgien seit dem Vortag aufgenommen wurde. Am Aufnahmetag war es zu einem generalisierten Krampfanfall gekommen. Vorerkrankungen bestanden nicht. In den Laboruntersuchungen fand sich eine Verbrauchskoagulopathie bei septischem Schock mit Entwicklung eines Multiorganversagens und Pneumokokkennachweis in der Blutkultur. Weiterhin zeigte sich eine Purpura fulminans im Bereich der gesamten Haut. Die Patientin verstarb innerhalb von 24 h nach Aufnahme auf der Intensivstation. Bei der Obduktion fand sich neben Einblutungen der Nebennieren und des Myokards eine Hypoplasie der Milz.
Eine Pneumokokkensepsis ist eine lebensbedrohliche Erkrankung, wobei neben anderen Risikofaktoren eine Hypo- oder Asplenie prädisponierend ist. In seltenen Fällen kann eine Pneumokokkensepsis die klinische Erstmanifestation einer bis dato unbekannten Hyposplenie sein. In diesem Zusammenhang muss die allgemeine Impfempfehlung gegen Pneumokokken für Patienten mit Risikofaktoren wie z. B. einer Hypo- oder Asplenie sowie für Patienten ab dem 60. Lebensjahr und im Kindesalter ab dem 2. Monat betont werden.
Abstract
We report a 52-year-old female patient admitted with fever, chills, and myalgias since the previous day. On the day of admission she had a generalized seizure. The patient had no previous illnesses. Laboratory investigations showed consumptive coagulopathy with clinical manifestations of shock and development of multiple organ failure. Pneumococci were detected in blood cultures. Furthermore the skin showed purpura fulminans all over. The patient died within 24 h after admission in the intensive care unit. On autopsy, in addition to adrenal and myocardial hemorrhages, hypoplasia of the spleen was found.
Fulminant pneumococcal sepsis is a life-threatening disease that occurs in patients with risk factors like splenic hypoplasia or asplenia. Sometimes a fulminant pneumococcal sepsis may be the first clinical manifestation of a hitherto unknown splenic hypoplasia. In this context the general recommendation of vaccination against pneumococci in patients with risk factors like splenic hypoplasia or asplenia, in patients older than 60, and in children from 2 months onward has to be emphasized.
