Contexte
La thrombolyse intraveineuse (TIV) par activateur tissulaire du plasminogène recombinant (rt-PA) est recommandée pour la prise en charge d’accident vasculaire cérébral (AVC) ischémique aigu. Nous avons évalué la faisabilité et la sécurité du traitement dans un contexte d’absence d’unité neurovasculaire.
Méthodes
Nous avons mené une étude rétrospective à partir des données informatisées de tous les patients ayant bénéficié d’un traitement par TIVau centre hospitalier de Cayenne entre avril 2014 et avril 2016. Les critères de sélection sont basés sur les résultats des études NINDS et ECASS III, hormis quelques situations discutables telles que l’âge >80 ans, la présence d’épilepsie, etc. Les différents délais entre le début des symptômes d’AVC, l’arrivée aux urgences et le traitement par rt-PA ont été calculés. Le score NIHSS (National Institute of Health Stroke Scale) a été utilisé à l’admission, puis pour l’évaluation des résultats cliniques 24 heures après la TIV et avant la sortie, ainsi que le mRs à la sortie. L’IRM cérébrale a été l’imagerie de choix à l’entrée et à 24 heures postthrombolyse. Mais, dans certains cas, le scanner cérébral avec ou sans produit de contraste a aussi été utilisé.
Résultats
Quarante-huit patients victimes d’AVC ischémiques aigus ont été inclus avec une prédominance masculine de 37 sur 48 soit 77 % et un âge moyen de 64 ans. Le temps écoulé entre la survenue de l’AVC au traitement rt-PA était de 202 min soit 3h22 et le délai de l’arrivée aux urgences jusqu’au début de la TIV est de 110 min (1h50). Vingt-six patients ont présenté une récupération clinique précoce avec une diminution du score NIHSS de quatre points à 24 heures, parmi lesquels 18 ont poursuivi leur amélioration jusqu’à la sortie. L’hémorragie intracérébrale post-TIV a été observée chez trois patients, tous asymptomatiques. Le nombre total de décès était de cinq patients soit 10 % dont trois avec une occlusion de l’artère cérébrale moyenne avec échec à la TIV, aggravation et décès, par défaut d’accès à la thrombectomie. Conclusion: Notre étude, réalisée sur un petit échantillon, démontre que la TIV d’AVC ischémique aigu est aussi faisable avec moins de risque en cas de nécessité et en absence d’unité neurovasculaire. Mais, ceci n’est réalisable qu’en présence de praticiens compétents et formés dans la prise en charge de la pathologie neurovasculaire.
Abstract
Background
Intravenous thrombolysis by recombinant tissue plasminogen activator (rt-PA) is recommended to treat an acute ischemic stroke.We evaluated the feasibility and safety of treatment in a context without stroke unit. Methods: We conducted a retrospective study using computerized data from all patients computerized data that received intravenous thrombolysis treatment at Cayenne Hospital Center between April 2014 and April 2016. The selection criteria are based on the results of the NINDS study and ECASS III, except some questionable situations such as higher age at 80 years, the presence of epilepsy, etc. Different time between the onset of stroke symptoms, emergency arrival and treatment with rt-PA were calculated. The National Institute of Health Stroke Scale (NIHSS) was used for admission and for the evaluation of clinical results 24 hours after intravenous thrombolysis and before discharge, and the mRs at the discharge. Brain MRI was imaging at the entrance and at 24 hours after thrombolysis. But in some cases, CT-scan with or without contrast was also used.
Results
48 patients with acute ischemic stroke were treated by the intravenous thrombolysis with a male predominance of 37 to 48 or 77 % and an average age of 64 years. The time from stroke onset to rt-PA treatment was 202 min or 3h22 and the time of arrival to the emergency until the beginning of the intravenous thrombolysis or “door-to-needle” was 110 min (1h50). 26 patients had early clinical recovery with a decrease in the NIHSS score of 4 points to 24 hours, of which 18 continued to improve to the exit. The intracerebral hemorrhage after thrombolysis was observed in three patients, all asymptomatic. The total number of deaths was 5 patients or 10 %, 3 with occlusion of the middle cerebral artery with failure to intravenous thrombolysis, aggravation and death, because there’s no thrombectomy.
Conclusion
Our study, performed on a small sample, demonstrates that the intravenous thrombolysis of acute ischemic stroke is feasible with less risk in case of necessity and in the absence of stroke unit. But, this is feasible only in the presence of competent practitioners and trained in the treatment of stroke disease
