Zusammenfassung
Die rituelle Beschneidung des äußeren weiblichen Genitales („female genital mutilation/cutting“, FGM/FGC) ist in vielen Regionen dieser Erde verbreitet und kulturell tief verwurzelt. Organisch hinterlässt dieses Ritual massive Form- und Funktionsbeeinträchtigungen der Urogenitalregion mit konsekutiven Miktions- und Menstruationsbeeinträchtigungen sowie Störungen der Sexualfunktion und Entbindung. Die Rekonstruktion des weiblichen Genitales stellt daher eine kurative und medizinisch indizierte operative Maßnahme dar, deren Inhalte sich nach der Radikalität der Beschneidung (Typ I–IV nach WHO) richten und gleichsam spezielle operative Techniken erfordern, um ein anatomisch-funktionelles Ergebnis zu erzielen. Die Bergung des Klitorisstumpfes, mikrochirurgische Rekonstruktion einer Glans clitoridis, präputiale Einbettung der Neoglans mittels „omega domed flap“ („OD-flap“) und die komplexe Vulvarekonstruktion mittels „anterior obturator artery perforator flap“ („aOAP-flap“) führen zu einer Normalisierung von Form und Funktion des äußeren weiblichen Genitales. Für betroffene Mädchen und Frauen leisten diese speziellen operativen Techniken damit einen effektiven Beitrag zur Überwindung von FGM.
Abstract
Ritual cutting of the external female genitalia (FGC), or female genital mutilation (FGM), is practised in many regions of the world and is culturally deeply embedded. This ritual leaves behind massive form and functional impairments of the urogenital region with subsequent impairments in micturition and menstruation as well as disorders of sexual function and parturition. Reconstruction of the female genitalia therefore represents a curative and medically indicated interventional measure. The content depends on the radicality of the FGM, types I-IV according to the World Health Organization (WHO) and simultaneously requires special operative techniques in order to achieve an anatomically functional result. The salvage of the clitoris stump, microsurgical reconstruction of the glans of the clitoris, preputial embedding of the neoglans with an omega domed flap (OD flap) and the complex vulvar reconstruction with an anterior obturator artery perforator flap (aOAP flap) lead to a normalization in form and function of the external female genitalia. For affected girls and women these special operative techniques therefore provide an effective contribution to overcome FGM.
