Résumé
Cette étude rapporte l’expérience de l’utilisation en routine pendant 15 mois d’un spectromètre de masse (SM) de type MALDI-TOF (MALDI Biotyper-Microflex®, Bruker Daltonics) sur un plateau technique de bactériologie traitant en moyenne 250 échantillons par jour. Les résultats de validation de méthode (répétabilité et reproductibilité) à partir de 40 souches de référence sont présentés. Sur les 22 bactéries à Gram négatif, deux souches ont été mal identifiées et deux autres n’ont permis qu’une identification du genre. Pour les bactéries à Gram positif, les 18 souches testées ont toutes été correctement identifiées.
L’intérêt de la SM pour le laboratoire est décrit et illustré à l’aide d’exemples cliniques. Une étude économique est également présentée.
Abstract
This study reports the experiment of the routine use for 15 months of a mass spectrometer (MS) type MALDI-TOF (MALDI Biotyper-Microflex®, Bruker Daltonics) in a medical microbiology centre of a private laboratory, analyzing an average of 250 bacteriology samples per day. The results of the method’s validation tests (repeatability and reproducibility), from 40 reference strains, are shown. From 22 Gram-negative bacteria, two strains have been misidentified and two others only on genus. For the Gram-positive bacteria, the 18 tested strains have all been correctly identified to the species with first scores on average all exceeding 1.90.
Finally, the benefit of the introduction of mass spectrometry is described with several examples of clinical results as well as an economic approach.
