Clinical observation of Zhen’ai needling method in Nei Jing (Classic of Internal Medicine) for children with allergic rhinitis accompanied by adenoid hypertrophy

《内经》振埃刺法治疗儿童变应性鼻炎伴腺样体肥大临床观察

Abstract

Objective

To observe the clinical efficacy of Zhen’ai needling method in Nei Jing (Classic of Internal Medicine) for children with allergic rhinitis (AR) accompanied by adenoid hypertrophy (AH).

Methods

A total of 74 children who met the screening criteria were divided into a Zhen’ai group and a control group by the random number table method, with 37 cases in each group. The control group was treated with acupuncture at Zusanli (ST 36), Hegu (LI 4), Yingxiang (LI 20), Juliao (ST 3), Yintang (GV 29), Shangxing (GV 23) and Baihui (GV 20). The Zhen’ai group added points of Zhen’ai needling method {Shanglianquan [Extra, located at 1 cun above Lianquan (CV 23)], Tianrong (SI 17) and Lieque (LU 7)} in addition to the points in the control group. The needles were retained for 30 min. The treatment was performed twice a week. The total nasal symptom score (TNSS), sino-nasal outcome test-20 (SNOT-20) and symptom scale for AH (SSAH) were assessed before and after 10 treatments. The clinical efficacy of the two groups was compared after treatment.

Results

During the treatment, 5 cases dropped out in the control group and 2 cases in the Zhen’ai group. After treatment, the total effective rate of the Zhen’ai group was 94.3%, versus 93.8% in the control group. There was no significant difference between the two groups (P>0.05). The markedly effective rate of the Zhen’ai group was 42.9%, versus 12.5% in the control group, and the difference was statistically significant (P<0.05). After treatment, there were significant intra-group differences in the scores of TNSS, SNOT-20 and SSAH in both groups (all P<0.05); the scores of SNOT-20 and SSAH in the Zhen’ai group were lower than those in the control group, and the differences between the groups were statistically significant (both P<0.05).

Conclusion

Both conventional acupuncture and conventional acupuncture plus Zhen’ai needling method can improve clinical symptoms of children with AR accompanied by AH; and conventional acupuncture plus Zhen’ai needling method has a better effect than conventional acupuncture in improving AH symptoms.

目的

观察《内经》振埃刺法治疗儿童变应性鼻炎(AR)伴腺样体肥大(AH)的临床疗效.

方法

符合入组 标准的患儿74 例, 按随机数字表法分为振埃组和对照组, 每组37 例. 对照组予以针刺足三里、合谷、迎香、巨 髎、印堂、上星和百会治疗, 振埃组在对照组取穴基础上加刺振埃刺法穴位(上廉泉、天容和列缺). 每次留针 30 min, 每周治疗2 次. 于治疗前及治疗10 次后进行鼻症状评分表(TNSS)、20 项鼻腔鼻窦结局评分表(SNOT-20) 和AH 症状量表(SSAH)评分. 治疗后比较两组临床疗效.

结果

治疗过程中, 对照组脱落5 例, 振埃组脱落2 例. 治疗后, 振埃组总有效率为94.3%, 对照组为93.8%, 两组总有效率差异无统计学意义(P>0.05); 振埃组显效率为 42.9%, 对照组为12.5%, 两组显效率差异有统计学意义(P<0.05). 治疗后, 两组TNSS、SNOT-20 和SSAH 评分均 与本组治疗前有统计学差异(均P<0.05); 振埃组NOT-20 及SSAH 评分均低于对照组, 组间差异均有统计学意义 (均P<0.05).

结论

常规针刺及常规针刺加振埃刺法穴位均可改善AR 伴AH 患儿的临床症状; 常规针刺加振埃 刺法穴位在改善AH 症状方面优于常规针刺.
