Le fratture di ginocchio possono essere classificate in base alla sede anatomica, al meccanismo traumatico e all’età del paziente.
In particolare, quest’ultimo è un fattore rilevante per l’inquadramento e il trattamento della frattura, nonché un indicatore delle possibili complicanze. Infatti, le fratture in età infantile non sono sovrapponibili a quelle dell’adulto o dell’anziano.
La tipologia di frattura più frequente nel bambino è il distacco condro-epifisario e la sua rilevanza clinica risiede nella delicatezza della cartilagine di accrescimento che, quando compromessa, può portare a difetti di crescita e a deviazioni assiali.
L’anziano invece si differenzia dall’adulto per un tessuto osseo più fragile e, di conseguenza, per un maggior rischio di frattura. Tale rischio è aumentato anche in conseguenza a un impaccio motorio e a una limitazione dell’autonomia funzionale dovuti all’artrosi che incrementano a loro volta il rischio di caduta e quindi di frattura.
Inoltre nell’anziano, dato l’importante aumento negli ultimi decenni delle indicazioni all’intervento di protesi di ginocchio, è di grande rilievo il fenomeno delle fratture periprotesiche.
Il ginocchio è un’articolazione complessa e le fratture che possono interessarlo comprendono un gruppo eterogeneo di lesioni, quanto a caratteristiche epidemiologiche e a problematiche clinico-terapeutiche, che necessita di un attento inquadramento iniziale.
Abstract
Fractures of the knee can be classified according to anatomic location, the mechanism of injury and the age of the patient.
In particular, the patient’s age is an important factor for the classification and treatment of fracture. In fact, fractures in childhood are not comparable to those of the adult and the elderly.
The most common fracture in childhood is the chondro-epiphyseal detachment. Its clinical relevance lies in the sensitivity of the growth plate which, when compromised, can lead to defects in growth and axial deviation.
Elder instead differs from the adult bone tissue for a more fragile and, consequently, to a greater risk of fracture. The risk is also increased as a consequence of a clumsiness and a functional limitation due to arthritis which in turn increase the risk of fall and fracture.
Also in elderly, given the significant increase in the last decades of the indications for knee replacement, it is of great importance the phenomenon of periprosthetic fractures.
The knee is a complex articulation. Fractures of the knee include a heterogeneous group of lesions in terms of epidemiological characteristics and clinical and therapeutic issues, which requires a careful initial classification.
