Refractory Erosive Oral Lichen Planus Associated with Hepatitis C: Response to Topical Tacrolimus Ointment

Abstract

Background

Erosive lichen planus is a painful and disabling disease that is frequently resistant to topical and systemic therapies. Current therapies are considered palliative rather than curative as many patients relapse after discontinuing treatment. An association has been reported between some cases of oral lichen planus (OLP) and chronic hepatitis C infection.

Objective

We report on a 51-year-old hepatitis C-positive man with corticosteroid refractory erosive lichen planus of the lip who had a rapid resolution of his lesions following a two-week course of topical 0.1% tacrolimus ointment. The patient remains symptom-free at one year post-treatment.

Conclusion

This case supports the safety and efficacy of topical tacrolimus in patients with steroid-refractory OLP associated with chronic hepatitis C.

Antécédents

Le lichen plan érosif est une maladie douloureuse et invalidante qui est souvent résistante aux thérapies topiques et systémiques. Les thérapies actuelles sont considérées comme palliatives plutôt que curatives puisqu’un grand nombre de patients font une rechute après l’interruption du traitement. Une association entre certains cas de lichen plan buccal et l’infection chronique à l’hépatite C a été rapportée.

Objectif

Nous rapportons le cas d’un homme de 51 ans atteint d’hépatite C qui souffrait également d’un lichen plan érosif de la lèvre, réfractaire aux corticostéroïdes. Une résolution rapide de ses lésions a été constatée à la suite d’un traitement de deux semaines à 0,1% de tacrolimus topique. Le patient demeure sans symptômes une année après la fin du traitement.

Conclusion

Ce cas est une preuve de l’innocuité et de l’efficacité du tacrolimus topique chez les patients souffrant de lichen plan buccal refractaire aux stéroïdes et associé à l’hépatite C chronique.
