Zusammenfassung
Im Vordergrund der Behandlung von Mittelhandfrakturen steht die funktionelle Wiederherstellung der Handfunktion. Die Wahl der zur Verfügung stehenden Therapiemaßnahmen ist neben der radiologischen Ausgangssituation abhängig vom klinischen Erscheinungsbild. In der Regel werden stabile und primär nicht dislozierte Frakturen, bei denen keine Rotationsfehler, eine Achsenabweichung oder eine Verkürzung vorliegen, einer frühfunktionellen Übungsbehandlung zugeführt. Instabile, dislozierte Frakturen oder Frakturen bei Vorliegen einer Gelenkbeteiligung werden in den meisten Fällen reponiert und osteosynthetisch stabilisiert. Eine Sonderstellung nehmen Frakturen des 1. Mittelhandknochens ein. Die in der Regel instabilen Basisfrakturen werden operativ behandelt. Dies gilt auch für Mittelhandfrakturen, bei denen begleitende Weichteilverletzungen vorliegen.
Abstract
The focus of the treatment of metacarpal fractures is the functional restoration of the hand function. The choice of available treatment measures depends on the clinical presentation in addition to the radiological initial situation. As a rule stable and primarily non-displaced fractures in which no rotational errors, axis deviation or shortening are present, are treated by early functional exercise. Unstable, displaced fractures or in the presence of joint involvement are in most cases repositioned and osteosynthetically stabilized. Fractures of the first metacarpal bone represent a special situation. The mostly unstable base fractures are surgically treated. This is also the case for metacarpal fractures in which accompanying soft tissue injuries are present.
