Zusammenfassung
Die klinische Relevanz einer Bradykardie wird nicht durch die willkürlich festgelegten Grenzwerte der Herzfrequenz, sondern durch die hervorgerufene Symptomatik bestimmt. Die Bradykardien können durch die Kompensationsmechanismen, insbesondere die Erhöhung des Schlagvolumens asymptomatisch bleiben. Fast der Hälfte der akuten Bradykardien liegt eine reversible Ursache zugrunde. Die Behandlung dieser Ursache stellt ein Fundament der Bradykardiebehandlung dar. Mit den intravenösen chronotropen Substanzen und der Anlage eines passageren Schrittmachers stehen effektive Therapiemaßnahmen zur Behandlung der akuten Bradykardien zur Verfügung.
Abstract
Clinical relevance of acute bradycardia is driven by symptoms and not primarily by the reported decreased heart rate. Bradycardias may remain asymptomatic especially due to compensatory mechanisms (in particular increase of left ventricular ejection fraction). Nearly half of acute bradycardias have a reversible cause. Detection of potential reversible bradycardia causes is therefore regarded as the cornerstone of bradycardia treatment in the emergency setting. Effective therapies for the treatment of acute bradycardia are available, including intravenous chronotropic drugs and pacemaker implantation.
