Zusammenfassung
In diesem Artikel werden die meisten derzeit oder in naher Zukunft verfügbaren Vorhofohrverschlusssysteme im Hinblick auf Handhabung und Aufbau beschrieben. Hierzu zählen das Watchman™-Device der Fa. Boston, das Amulet™-Device der Firma Abbot, das Lambre™-Device der Firma Lifetec, das Occlutech®-Device der Fa. Occlutech® sowie das WaveCrest®-Device der Firma Biosense Webster (vorher Coherex). Die Systeme unterscheiden sich im Konzept des Verschlusses des linken Vorhofohrs. Ein Teil der Systeme wird in das Vorhofohr eingebracht und schließt mit dem Eingang des Vorhofohrs ab. So bestehen Watchman™- und Occlutech®-Systeme aus einer einzigen Einheit und werden komplett implantiert, andere haben zusätzlich eine Scheibe (Diskus), der als Deckel auf den Eingang des Vorhofrohrs gelegt wird. Beim Amulet™- und Lambre™-Device wird der zur Fixierung bestimmte Teil primär implantiert und dann die abdichtende Scheibe auf dem Eingang platziert. Beim WaveCrest®-System wird zuerst der abdichtende Teil des Devices im Eingang des linken Vorhofohrs platziert und im Anschluss der fixierende Teil des Ankersystems in den Korpus des linken Vorhofohrs (LAA) ausgefahren.
Abstract
In this article, most of the present or available in the near future left atrial appendage (LAA) occlusion devices are discussed in terms of structure of the devices and principle technique for implantation. Thus, the Watchman™ device from Boston, the Amulet™ device from Abbot, the Lambre™ device from Lifetech, the Occlutech® device from Occlutech®, and the WaveCrest® device from Biosense Webster (former Coherex) are discussed. The systems differ in terms of the concept of occlusion. Several devices consist of only one part (Watchman™ and Occlutech®) that is implanted in the body, ending up in the orifice of the LAA. Others like the Amulet™ or Lambre™ device have an additional disc covering the orifice of the LAA from the left atrial side. The distal anchoring system is implanted first in the body of the LAA; thereafter, the disc is placed upon the orifice. With the WaveCrest® device, covering of the orifice is performed as the first step followed by rolling in the anchoring system to fix the device to the body of the LAA.
