Summary
A few weeks after orthotopic heart transplantation, a male adolescent developed atrial arrhythmias of the donor heart due to an atypical recipient atrial flutter with a recipient-to-donor transatrial conduction resulting in an absolute arrhythmia. Under medication with propafenone, the atrial flutter of the donor heart could be terminated with cardioversion.
Zusammenfassung
Ein 14-jähriger Patient mit ausgeprägter dilatativer Kardiomyopathie entwickelte wenige Wochen nach orthotoper Herztransplantation eine Kreislaufinstabilität durch eine absolute Arrhythmie bei Vorhofflattern. Als wahrscheinlichste Ursache des Vorhofflatterns im Spenderorgan wird eine vom Empfänger-Vorhof übergeleitete atriale Arrhythmie angenommen. Unter einer Medikation mit Propafenon konnte das Vorhofflattern des Spenderorgans durch Kardioversion terminiert werden.
