Morphologische Grundlage des Sellschen Irritationspunktes für das Iliosakralgelenk

Zusammenfassung

Dem Iliosakralgelenk (ISG) des Menschen kommen wichtige Funktionen bei der Statik und Dynamik des Bewegungsapparates zu. Beim ISG-Syndrom hat Sell einen Irritationspunkt in der Regio glutea beschrieben. Wir sind mit morphologischen Methoden der Frage nachgegangen, welche Strukturen die am Irritationspunkt beschriebene lokale Konsistenzerhöhung verursachen. Es zeigte sich, dass die Ansatzportion des M. piriformis unterhalb des auf der Haut markierten Irritationspunkts im Spatium subglutaeum gelegen ist. Das ISG und der M. piriformis werden aus den Rückenmarkssgementen S1 und S2 versorgt. Beide Strukturen sind deshalb als Stationen eines Reflexbogens verknüpft. Unsere Befunde machen es sehr wahrscheinlich, dass die beim ISG-Syndrom unter dem M. glutaeus maximus palpable Gewebsverhärtung auf eine reflektorische Verspannung des M. piriformis zurückzuführen ist.

Abstract

The sacroiliac joint is involved in transmitting weight from the vertebral column to the lower limbs. A syndrome of this joint results according to Sell (1970) in a local increase of tissue consistency that can be felt underneath the gluteus maximus muscle. To identify the morphological basis of this swelling a dye was injected into the structures located underneath this irritation point and the muscles, blood vessels, and nerves were dissected in the subgluteal region. The piriformis muscle was found to cross the line projecting ventrally from the irritation point. Since both the piriformis muscle and the sacroiliac joint are supplied by identical spinal nerves S1 and S2 it is concluded that a contraction of the piriformis muscle is the morphological basis of the circumscribed swelling palpable beneath the gluteus maximus muscle.
