Chiropractic of cervical spine – Normal results of x-ray examination despite tumourous destruction of cervical joints

Summary

Severe complications after chiropractic treatments rarely occur. They happen mostly when manipulating the atlantoaxial and the craniovertebral articulations or when not attending to the contraindications like partial dislocation and hemivertebra. Therefore, x-ray pictures from the part of the vertebral region to be treated are inevitable and likewise the medical scientific associations demand them uniformly to minimize the risks. Nevertheless, our case-report shows also inconspicuous radiological findings do not exclude local tumor infestation of the bones with possible deleterious results for the patient treated by the chiropractor.

We report the case of an 45-year old male patient treated with chiropractic of the cervical spine relying on normal results of the x-ray examination. After treatment, there were no complications till nowadays. Surprisingly, computed tomography (CT) and nuclear magnetic resonance (MRI), which were made two months later due to refractory pains, revealed a plasma cell tumor (pleomorphic T-cell lymphoma) infiltrating the upper three cervical joints.

In conclusion, a careful tumor anamnesis is essential, especially in the consulting hour of a chiropractor. Moreover, the possibility of a relapse of the tumor with bone metastasis should be included in consideration, when there are any information about malignant neoplasm's in the history. Furthermore, CT and MRI are more suitable in detecting bone metastasis than the radiography. We want to point out, also inconspicuous radiological findings do not release the chiropractor from the liability to conscientious medical care. To minimize the risks, the authors are pleading for a good mastery of the chiropractice technique and particularly for the application of the so-called soft chiropractic techniques by attending to the upper cervical spine.

Zusammenfassung

Schwere Komplikationen treten nach chirotherapeutischen Behandlungen insgesamt sehr selten auf. Sie ereignen sich überwiegend bei Manipulationen im Kopfgelenksbereich oder bei Nichtbeachtung von Kontraindikationen wie Subluxation und Halbwirbel. Die Anfertigung von Röntgenaufnahmen des zu behandelnden Wirbelsäulenabschnitts ist vor der Chirotherapie unabdingbar und wird auch gleichlautend von allen manualmedizinischen Fachgesellschaften zur Risikominimierung gefordert. Unser Fallbeispiel zeigt: Auch ein unauffälliger radiologischer Befund schließt letztendlich einen lokalen Tumorbefund mit potentiell deletären Folgen für den chirotherapeutisch Behandelten nicht aus.

Geschildert wird der Fall eines 45 jährigen Patienten, bei dem bei unauffälligem radiologischen Befund der HWS eine vorsichtige Chirotherapie durchgeführt wurde. Komplikationen traten nach der Behandlung nicht auf. Die wegen Therapieresistenz zwei Monate später angefertigten CT- und MRT-Untersuchungen ergaben überraschenderweise eine Destruktion der obersten drei HWK durch ein Plasmozytom (anaplastisches T-Zell-Lymphom).

Gefolgert wird, in der manualtherapeutischen Sprechstunde ist eine sorgfältige Anamneseerhebung besonders wichtig. Bei Hinweis auf ein Malignom in der Vorgeschichte sollte an die Möglichkeit eines Rezidivs mit Knochenmetastasierung gedacht werden. Offensichtlich sind CT und MRI dem konventionellen Röntgen beim Nachweis von Knochenmetastasen überlegen. Auch ein unauffälliges Röntgenbild entbindet den Manualtherapeuten nicht von seiner Pflicht zur sorgfältigen Behandlung. Zur Risikominderung plädieren die Autoren für eine gute Beherrschung der chirotherapeutischen Technik und für die Anwendung von sogenannten „weichen“ Techniken an der oberen Halswirbelsäule.
