Hintergrund
Kombinierte ipsilaterale Verletzungen des medialen und lateralen Schultergürtels sind selten, und es gibt nur wenige Fallberichte, die deren erfolgreiche Behandlung beschreiben. Diese Verletzungen sind komplex und beeinflussen sowohl knöcherne als auch weichgewebige Strukturen. Um ein akzeptables klinisches Ergebnis und anatomische Verhältnisse sicherzustellen, sollten beide Komponenten in der Therapie berücksichtigt werden.
Fall
Vorgestellt wird ein Patient mit einer kombinierten Verletzung des Schultergürtels nach einem Fahrradunfall. Operativ erfolgten eine offene Reposition und interne Fixation der medialen Klavikulafraktur. Additiv wurde eine arthroskopisch-navigierte Rekonstruktion der korakoklavikulären Bänder durchgeführt. Die Nachuntersuchungen ergaben anatomische Verhältnisse und eine gute klinische Mobilität und Stabilität im gesamten Schultergürtel.
Schlussfolgerung
Bei komplizierten Verletzungen des Schultergürtels kann eine gute Mobilität und Stabilität erreicht werden, wenn sowohl die knöchernen als auch die weichgewebigen Komponenten in der chirurgischen Therapie berücksichtigt werden.
Abstract
Background
Combined ipsilateral injuries of the medial and lateral shoulder girdle are rare and reports on successful treatment are limited. These injuries are complex, involving both bony and soft tissue structures (i.e., the clavicle and the acromioclavicular joint). For successful management both structures should be considered to ensure an acceptable clinical outcome and anatomical relations.
Case report
We report a patient with a complex combined injury of the shoulder girdle after a bicycle accident. We performed open reduction and internal fixation of the medial clavicle fracture. In addition, an arthroscopically navigated reconstruction of the coracoclavicular ligaments was carried out. The follow-up examination showed good anatomical relations and a good clinical mobility and stability of the complete shoulder girdle.
Conclusion
In complex injuries of the shoulder girdle, stability and full range of motion of the shoulder can be achieved when both bony and soft tissue structures are taken into consideration in the surgical treatment.
