Zusammenfassung
Bei Kindern mit der erblichen Nierenerkrankung Alport-Syndrom zeigt sich die überragende Bedeutung, die der Abklärung der (Mikro‑)Hämaturie im Kindesalter zukommt, um eine frühzeitige nephroprotektive Therapie zu ermöglichen. Diese verzögert nicht nur die Nierenerkrankung entscheidend, sondern verbessert auch die Lebenserwartung des Betroffenen. Das Alport-Syndrom führt, verursacht durch Genvarianten in den Typ-IV-Kollagen-Genen, zur progredienten Nierenfibrose. Früher wurde die Hämaturiesymptomatik heterozygoter Anlageträger fehlerhaft als familiäre benigne Hämaturie bezeichnet. Der Krankheitsprozess bei diesen heterozygoten Anlageträgern nimmt jedoch häufig keinen benignen Verlauf, daher wird die Symptomatik mittlerweile auch unter der Diagnose „Alport-Syndrom“ zusammengefasst. Das Alport-Syndrom ist bei früher Diagnose gut behandelbar; das Nierenversagen kann um viele Jahre verzögert werden. Aufgrund der Therapiemöglichkeiten sollte die Diagnose bei jedem Kind, auch heterozygoten Anlageträgern, frühzeitig molekulargenetisch gestellt werden. Oft erfolgt sie allerdings erst im jungen Erwachsenenalter, wenn die Nierenfunktion schon eingeschränkt ist.
Der vorliegende Beitrag zeigt dem Pädiater am Beispiel des Alport-Syndroms, wissenschaftlich begründet, die Möglichkeiten einer an die Krankheitsstadien adaptierten nephroprotektiven Therapie auf: (1) Bedeutung, Zeitpunkt und Methoden der Frühdiagnose, (2) Art und Häufigkeit der Kontrolluntersuchungen, insbesondere bei Hämaturie, (3) optimaler Therapiebeginn, (4) supportive Maßnahmen, (5) Therapieeskalation bei zunehmender Proteinurie sowie (6) Therapiemöglichkeiten und -ziele bei Sekundärkomplikationen. Ab 2019 können in Deutschland Jugendlichen mit Alport-Syndrom zudem neue Phase-II- und Phase-III-Therapiestudien angeboten werden.
Abstract
In children with the hereditary kidney disease Alport syndrome, the extreme importance of clarification of (micro)hematuria in childhood in order to enable an early nephroprotective treatment can be shown. This not only decisively delays the kidney disease but also improves life expectancy. Children with Alport syndrome develop progressive renal fibrosis due to genetic variants in type IV collagen genes. In the past the symptomatic hematuria symptoms in heterozygous carrier patients was misclassified as familial benign hematuria; however, the disease process by these heterozygous carrier patients often does not have a benign course of the disease. Therefore, all homozygous and heterozygous patients are now summarized under the diagnosis Alport syndrome. If diagnosed early Alport syndrome can be well treated. Renal failure can be delayed by many years and life expectancy can be improved. Because of the available treatment options, the diagnosis should be made as soon as possible in every affected child by molecular genetic testing. Unfortunately, the diagnosis is often made too late at the age of early adolescence when the renal function is already severely impaired.
This article aims to show the pediatrician the scientifically founded possibilities for disease stage-adapted nephroprotective treatment: (1) relevance, timing and methods for early diagnosis, (2) mode and frequency of control investigations, especially in children with hematuria, (3) optimal start of treatment, (4) supportive measures, (5) escalation of treatment if proteinuria increases and (6) therapeutic options and goals in secondary complications. Additionally, in 2019 new phase II and III interventional studies with new medications can be offered in Germany to adolescents with Alport syndrome.
