Zusammenfassung
Die Leitlinien zur Implantation von Defibrillatoren beinhalten aufgrund der Ergebnisse von MADIT II, Companion und SCD-HeFT Empfehlungen zur Primärprävention des plötzlichen Herztods. Die drei wesentlichen Risikofaktoren für eine erhöhte Mortalität durch ventrikuläre Tachykardien sind ein vorangegangener Herzinfarkt, eine eingeschränkte linksventrikuläre Funktion und die chronische Herzinsuffizienz. An Fallbeispielen wird die Entscheidung zur Implantation eines Defibrillators oder biventrikulären Defibrillators diskutiert.
Abstract
The guidelines for the implantation of cardioverter defibrillators recommend the primary prevention of sudden cardiac death based on the results of MADIT II, Companion and SCD-HeFT. The main risk factors for ventricular arrhythmias are previous myocardial infarction, depressed left ventricular function, and chronic heart failure. The presented case reports demonstrate the indication for a defibrillator or biventricular defibrillator as a basis of clinical pathways.
