Grundlagen: Die Pathophysiologie der gastroösophagealen Refluxkrankheit ist multifaktoriell. Nur eine genaue Abklärung mit Endoskopie und Biopsie, Manometrie und 24-h-pH-Metrie kann eine erfolgreiche Therapie gewährleisten.

Methodik: Die laparoskopische Antirefluxchirurgie stellt ein technisch anspruchsvolles Verfahren dar, welches nur von laparoskopisch erfahrenen Operateuren ausgeführt werden sollte. Komplikationen können einerseits durch sorgfältige Patientenauswahl und andererseits durch geeignete chirurgische Technik minimiert werden.

Ergebnisse: Zuletzt publizierte Ergebnisse zeigen, daß die laparoskopische Fundoplikatio in der Version des „Floppy Nissen” bzw. eines „Toupet” eine ausgezeichnete Ergebnisqualität bietet und in Zentren die Problemrate deutlich unter 10% liegt.

Schlußfolgerungen: Das laparoskopische Vorgehen in der Antirefluxchirurgie ersetzt zunehmend die offenen Operationsverfahren und ist daran, sich als Standard zur chirurgischen Behandlung der gastroösophagealen Refluxkrankheit zu etablieren.

Background: The pathophysiological aspects of gastroesophageal reflux disease are multifactorial. Therefore an exact preoperative assessment including endoscopy and biopsy, manometry and 24-h pH-monitoring is essential for a successful therapy.

Methods: Laparoscopic antireflux surgery is technically demanding and should be done only by surgeons who are firm in laparoscopic surgery.

Results: Complications can be minimized by carefully selecting patients and using an appropriate technique. Follow-up data are published and indicate that the “Floppy Nissen” and “Toupet” fundoplication can offer a good medical outcome with less than 10% of problems in surgical centers.

Conclusions: Laparoscopic antireflux surgery has replaced traditional operations and may become a “standard” 120 surgical therapy of gastroesophageal reflux disease.
