Summary
In the development of PIA II, all of the 567 patients were included who had been operated on for intra-abdominal infections. Discriminant analysis was used to rank the variables used according to their power to discriminate between the target variables SURVIVOR-NONSURVIVOR and to attribute a discrimination factor to each. The variables used and the scores were: male sex (−0.61), age over 60 years (−0.38), history of infection or more than 48 h (−0.40), congestive heart failure (−0.47), insulin-dependent diabetes mellitus (−1.36), leukocyte count of less/more than 5000/26 000/nl (−2.47), creatine over 4 mg/dl (−0.75), diffuse peritonitis (−0.34), peritonitis due to mesenteric infarction (−4.19), due to intestinal strangulation (−2.74), due to perforated peptic ulcer (0.49), due to perforated appendix (0.62), due to perforation of colon (0.77), and the constant (2.02). The midpoint is zero and a score of ± 1.315 showed the probability of survival or nonsurvival (97% correct prediction). The PIA II classified 89% of all test group patients correctly. Validation in the unknown control group showed a correct classification of 81.4%.
Zusammenfassung
Anhand unseres Peritonitiskrankengutes von 567 Patienten wurde über die Diskriminanzanalyse der Einfluss verschiedener, bei Erstoperation erhobenen Patientenvariablen auf die Zielvariablen LEBT-TOT mathematisch exakt definiert und daraus Peritonitisindex mit folgenden Variablen (score) entwickelt: Alter über 60 Jahre (−0,61), männlich (−0,61), Anamnese über 24 h (−0,40), Herzinsuffiziens (−0,47), insulinbedürftiger Diabetes mellitus (−1,36), Kreatinin über 4 mg/dl (−0,75), Leukocyten unter/über 5000 und 26 000/nl (−2,47), Ausbreitung diffus (−0,34), Durchwanderung bei Ileus (−2,74), Mesenterialverschluss (−4,19), Ulcusperforation Magen/Duodenum (0,49), Ursprung Colon (0,77), Konstante (2,02), Mittelpunkt ist Null; bei einem Score von −1,315 ist die Wahrscheinlichkeit zu sterben 97,5%. Die Klassifikationsgenauigkeit des Index beträgt 87,4%. Die Validierung am unbekannten Kontrollkollektiv ergab 81,4% richtig klassifizierte Fälle.
