Summary

Intravascular ultrasound (IVUS) is a clinically useful tool that provides cross-sectional images of the coronary arterial lumen and wall. Diagnostic applications of IVUS include the evaluation of left main coronary artery disease and of other ambiguous lesions on angiography particularly at bifurcations. IVUS is extremely useful for the assessment of coronary vasculopathy in cardiac transplant recipients, and it can also help to characterize other abnormalities such as syndrome-X, myocardial bridging and coronary artery spasm. Used as a complement to angiography, IVUS can optimize the monitoring of percutaneous coronary interventions and lead to a smaller residual plaque burden. It is also the best tool to assess stent deployment. IVUS allows the precise evaluation of the effects of approaches such as antioxidation or brachytherapy for the prevention of restenosis. It also represents the ideal tool for assessing regression of atherosclerosis. Three-dimensional reconstruction, elastography and imaging guidewires are some of the recent advances in the field of intravascular ultrasound.
